健儿清解液治疗儿童上呼吸道感染的系统评价和Meta分析  

Systematic Review and Meta-Analysis of Jianer Qingjie Liquid in the Treatment of Acute Upper Respiratory Tract Infection in Children

摘　　要：目的:系统评价健儿清解液治疗儿童上呼吸道感染的有效性、安全性和经济性。方法:计算机检索中国期刊全文数据库(CNKI)、中文科技期刊全文数据库(VIP)、万方数据知识服务平台、中国生物医学文摘数据库(CBM)、Pubmed、Embase和Cochrane library,检索时间均为建库至2021年8月。采用Cochrane风险偏倚评估工具评价纳入研究的质量,使用统计软件Revman 5.4进行Meta分析。结果:共纳入13项RCT,共计1980例患儿,其中健儿清解液组977例,其他药物治疗组1003例。Meta分析结果显示:健儿清解液联合常规治疗可提高疾病总有效率[RR=1.19,95%CI(1.03,1.39),P=0.02],且单用健儿清解液疗效均优于利巴韦林[RR=1.25,95%CI(1.15,1.36),P<0.00001]和清热解毒类中成药[RR=1.20,95%CI(1.11,1.29),P<0.00001],但与清热导滞类中成药(豉翘清热颗粒)相比,后者治疗总有效率更高[RR=0.81,95%CI(0.73,0.89),P<0.0001]。与常规治疗组和利巴韦林组相比,健儿清解液组能明显缩短退热时间[MD=-1.18,95%CI(-1.43,-0.92),P<0.00001]。健儿清解液组与其他治疗组不良反应发生率差异无统计学意义[RR=0.33,95%CI(0.01,9.28),P=0.51]。结论:健儿清解液治疗急性上呼吸道感染有较好疗效,受纳入研究的数量和质量限制,仍需开展更多高质量随机对照临床试验以进一步验证。建议在有效性的基础上,开展更多关于经济性的研究。Objective To systematically evaluate the efficacy,safety and economy of Jianer Qingjie liquid in the treatment of children with acute upper respiratory tract infection.Methods CNKI,VIP,WanFang Data,CBM,Pubmed,Embase and Cochrane Library were searched from the date of establishment to August 2021.Cochrane risk bias assessment tool was used to evaluate the quality of the included studies,and Revman 5.4 was used for Meta-analysis.Results A total of 13 RCTs involving 1980 patients were included,while the group of JEQJ involving 977 patients,other drug involving 1003 patients.The results of meta-analysis showed that Jianer Qingjie solution combined with conventional treatment could improve the total effective rate[RR=1.19,95%CI(1.03,1.39),P=0.02],and the efficacy of Jianer Qingjie liquid alone was better than ribavirin[RR=1.25,95%CI(1.15,1.36),P<0.00001]and traditional Chinese patent medicine[RR=1.20,95%CI(1.11,1.29),P<0.00001],but it was lower than that of Chiqiao Qingre granule[RR=0.81,95%CI(0.73,0.89),P<0.0001].Compared with conventional treatment group and ribavirin group,JEQJ group can shorten the antipyretic time obviously[MD=-1.18,95%CI(-1.43,-0.92),P<0.00001].There was no significant difference in the incidence of adverse reactions between JEQJ group and other treatment groups[RR=0.33,95%CI(0.01,9.28),P=0.51].Conclusion Jianer Qingjie liquid has a good efficacy in the treatment of acute upper respiratory tract infection.Limited by the number and quality of the included studies,more high-quality randomized controlled clinical trials need to be carried out for further verification.It is suggested to carry out more research on economy on the basis of effectiveness.
