维拉帕米膀胱灌注对PKRP后预防OAB及快速康复的临床分析

摘　　要：目的 探讨维拉帕米(Verapamil)膀胱灌注预防经尿道前列腺等离子电切术(PKRP)后膀胱过度活动症(OAB)临床症状的有效性、安全性。方法 回顾性分析2015年2月-2019年12月酒泉市人民医院收治的116例临床资料完整的PKRP术后患者,设为研究组(62例)和对照组(54例),比较分析2组患者术后72 h内膀胱区痉挛性疼痛、诱发血性灌洗液、出现急迫性尿失禁的例数,比较患者术后肠功能恢复时间、下地活动时间、拔除尿管时间。结果 与对照组比较,研究组患者术后膀胱区痉挛性疼痛例数、诱发血性灌洗液的发生例数、急迫性尿失禁的例数明显优于对照组,差异有统计学意义(P<0.05,χ^(2)=0.217);术后研究组患者肠功能恢复时间、下地活动时间、拔除尿管时间均少于对照组,差异有统计学意义(均P<0.01),治疗期间研究组未见明确不良反应。结论 Verapamil膀胱灌注对预防深部静脉血栓及肺部疾患等有明显的优越性,令患者快速康复,是一种安全有效的治疗方法。Objective To study the effectiveness and safety of bladder irrigation with Verapamil in the preventive clinical symptoms of overactive bladder(OAB) and rapid rehabilitation after transurethral bipolar plasmakinetic resection of prostate(PKRP). Methods Clinical data of 116 patients(62 cases in the study group,54 cases in the control group) with benign prostatic hyperplasia who were admitted to People’s Hospital of Jiuquan from February 2015 to December 2019 were analyzed retrospectively. Cases of bladder spasm, cases of washing solution turning bloody, cases of urgent urinary incontinence, time of anal exhaust, time of getting out of bed and time of catheter indwelling were compared between the two groups. Results A comparison between the control and the study group showed a significant difference in the frequency of OAB, cases of bladder spasm, cases of washing solution turning bloody, cases of urgent urinary incontinence after treatment(P < 0.05, χ^(2)= 0.217), while in the study group, time of anal exhaust, time of getting out of bed and time of catheter indwelling were significantly superior to those of the control group, the difference was statistically significant(P < 0.01), without obvious adverse reaction. Conclusion Verapamil bladder had obvious advantages in preventing deep vein thrombosis and pulmonary diseases. Resulting into rapid recovery of patients, it can be a safe and effective method of treatment.
