摘　　要：目的 旨在评估出院时的红细胞体积分布宽度(RDW)对慢性阻塞性肺疾病急性加重(AECOPD)患者1年内再入院情况的预测价值。方法 回顾性收集兰州大学第一医院2018年1月-2021年1月因AECOPD住院的291例患者。在电子病历系统中采集患者的性别、年龄、吸烟史、入院血常规、肺功能参数、住院天数、出院时的RDW及出院后1年内急性加重再入院次数等资料。结果 RDW与嗜酸性粒细胞计数呈负相关关系(r=-0.16,P<0.05),与中性粒细胞/淋巴细胞比值呈正相关关系(r=0.17,P<0.05)。RDW升高组的1年内再入院率显著大于RDW未升高组的。RDW的曲线下面积为0.676,可较好地评估AECOPD肺功能的恶化情况。结论 出院时RDW是指示AECOPD患者疾病严重程度和预测其1年内再入院的潜在生物指标。Objective To evaluate the predictive value of red blood cell volume distribution width(RDW) at discharge for 1-year readmission in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods The data of 291 patients hospitalized for AECOPD in the First Hospital of Lanzhou University from January 2018 to January 2021 were retrospectively collected in the electronic medical record system, such as gender, age, smoking history, admission blood routine, pulmonary function parameters,length of hospital stay, RDW at discharge and the number of acute exacerbation readmissions within one year after discharge. Results RDW was negatively correlated with eosinophil count(r =-0.16, P < 0.05), and positively correlated with neutrophil/lymphocyte ratio(r = 0.17, P < 0.05). The 1-year readmission rate of patients with increased RDW was significantly higher than that of patients without increased RDW(P < 0.05). The area under the curve of RDW was 0.676. RDW could better evaluate the deterioration of pulmonary function in AECOPD. Conclusion The level of RDW at discharge is a potential biological indicator telling about the disease severity of AECOPD patients and to predicting 1-year readmission of AECOPD patients.
