二甲双胍联合氯米芬配合生活方式干预对PCOS患者的疗效研究

摘　　要：目的:探讨二甲双胍(MF)联合氯米芬(CC)配合生活方式干预对多囊卵巢综合征(PCOS)患者的疗效。方法:选取2018年10月—2020年10月本院收治的76例PCOS患者作为研究对象,采用随机数字表法分为对照组及观察组,每组38例。对照组给予MF联合CC治疗,观察组于对照组基础上配合生活方式干预治疗。比较两组排卵效果[成熟卵泡数目、三经线阳性率及宫颈黏液评分]、生殖内分泌指标[雌二醇(E2)、促卵泡刺激素(FSH)、黄体生成素(LH)及睾酮(T)]、脂糖代谢参数[总胆固醇(TC)、三酰甘油(TG)、胰岛素抵抗指数(HOMA-IR)、空腹血浆胰岛素(FINS)]及血管活性因子水平[血管内皮生长因子(VEGF)、血清血管紧张素Ⅱ(AT-Ⅱ)]。结果:两组三经线阳性率比较,差异无统计学意义(P> 0.05);观察组成熟卵泡数目及宫颈黏液评分均高于对照组,差异有统计学意义(P <0.05);观察组E2、FSH、LH、T、TC、TG、HOMA-IR、FINS、VEGF、AT-Ⅱ水平低于对照组,差异有统计学意义(P <0.05)。结论:MF联合CC配合生活方式干预治疗PCOS患者的排卵效果良好,可有效改善生殖内分泌功能、脂肪代谢功能,且可调节血管活性因子。Objective: To investigate the effect of metformin(MF) combined with clomiphene(CC) combined with life intervention on the efficacy and vasoactive factors in patients with polycystic ovary syndrome(PCOS). Methods: A total of 76 patients with PCOS admitted to our hospital from October 2018 to October 2020 were enrolled in the study. They were divided into control group and observation group by random number table method. The control group was treated with MF combined with CC. On the basis of the control group, the observation group was treated with life intervention. The ovulation effects [number of mature follicles, positive rate of triplet and cervical mucus score], reproductive endocrine indexes [estradiol(E2), follicle stimulating hormone(FSH), luteinizing hormone(LH) and testosterone(T)], lipid and glucose metabolism parameters [total cholesterol(TC), triglyceride(TG), insulin resistance index(HOMA-IR), fasting plasma insulin(FINS)]and vascular activity factor levels [ vascular endothelial growth factor(VEGF), serum angiotensin II(AT-II)] were compared between the two groups. Results: There was no statistically significant difference in the positive rate of triple meridian between the two groups(P>0.05). The number of mature follicles and cervical mucus score in the observation group were higher than those in the control group, and the difference was statistically significant(P<0.05). The levels of E2, FSH, LH,T, TC, TG, HOMA-IR, FINS, VEGF and AT-II in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05). Conclusion: MF combined with CC combined with life intervention in the treatment of PCOS patients has good ovulation effect, which can effectively improve reproductive endocrine function and fat metabolism function, and can regulate vasoactive factors.
