甲苯磺酸瑞马唑仑联合艾司氯胺酮静脉麻醉在内镜下逆行胰胆管造影术中的应用

摘　　要：目的 探讨甲苯磺酸瑞马唑仑联合艾司氯胺酮静脉麻醉在内镜下逆行胰胆管造影术中的应用效果。方法 选取2020年9月-2021年9月我院行内镜下逆行胰胆管造影术的82例患者为研究对象,根据麻醉药物不同分为对照组40例和观察组42例。两组术中均行静脉麻醉,对照组给予右美托咪定联合丙泊酚,观察组给予甲苯磺酸瑞马唑仑联合艾司氯胺酮,比较两组麻醉前、内镜置入10 min后生命指标、术中呼吸循环抑制情况及不良反应情况。结果 观察组内镜置入10 min后MAP、HR、RR、SpO2水平高于对照组,差异有统计学意义(P<0.05);观察组心动过缓、低血压、SpO2下降、呼吸抑制发生率低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论 对于接受内镜下逆行胰胆管造影术患者,甲苯磺酸瑞马唑仑联合艾司氯胺酮有着理想的麻醉可控性,且对患者呼吸循环影响较小,安全性高,有利于术后快速转醒。Objective To investigate the application effect of remazolam tosylate combined with esketamine intravenous anesthesia in endoscopic retrograde cholangio-pancreatography.Methods A total of 82 patients who underwent endoscopic retrograde cholangio-pancreatography in our hospital from September 2020 to September 2021 were selected as study subjects.According to different anesthetic drugs,they were divided into control group(40 cases) and observation group(42 cases).Intravenous anesthesia was performed in both groups.The control group was given dexmedetomidine combined with propofol,and the observation group was given remazolam tosylate combined with esketamine.The life indicators before anesthesia and 10 min after endoscopic placement,intraoperative respiratory and circulatory inhibition and adverse reactions were compared between the two groups.Results The levels of MAP,HR,RR and SpO2in the observation group were higher than those in the control group at 10 min after endoscopic implantation,and the differences were statistically significant(P0.05).Conclusion For patients undergoing endoscopic retrograde cholangio-pancreatography,remazolam tosylate combined with esketamine has ideal anesthesia controllability,with less impact on patients’ respiratory circulation,high safety,and is conducive to rapid postoperative awakening.
