血栓弹力图在创伤性凝血病患者临床输血指导中的应用

摘　　要：目的探讨血栓弹力图在创伤性凝血病患者临床输血指导中的应用价值。方法选取2018年10月至2020年8月于本院应用血栓弹力图(TEG)检测指导临床输血的53例创伤性凝血病患者纳入观察组,另选取同期应用常规凝血检测指导临床输血的53例创伤性凝血病患者纳入对照组。比较TEG与常规凝血检测对创伤性凝血病低凝状态的检出结果,比较两组血浆、红细胞、冷沉淀、血小板输注量及输血相关并发症发生率。结果TEG对创伤性凝血病患者低凝状态的检出率为94.34%,高于常规凝血检测的81.13%,差异有统计学意义(P<0.05)。观察组输注血小板、血浆、冷沉淀、红细胞输注量均少于对照组,差异有统计学意义(P<0.05)。两组输血相关并发症发生率比较差异无统计学意义。结论血栓弹力图在创伤性凝血病患者临床输血指导中的应用效果较好,可优化输血方案,减少输血相关并发症的发生,提高输血治疗的安全性,对指导临床早期输血治疗具有一定的积极意义。Objective To explore the application value of thromboelastography in clinical blood transfusion guidance of patients with traumatic coagulopathy.Methods 53 patients with traumatic coagulopathy who underwent clinical blood transfusion guided by thromboelastography(TEG)in our hospital from October 2018 to August 2020 were selected as the observation group,and 53 patients with traumatic coagulopathy who used routine coagulation testing to guide clinical blood transfusion during the same period were selected as the control group.The results detection of coagulation state in traumatic coagulopathy by TEG and routine coagulation test were compared,and the blood plasma,erythrocyte,cryoprecipitate,transfusion volume of platelet and the incidence of blood transfusion-related complications were compared between the two groups.Results The detection rate of TEG in patients with traumatic coagulopathy was 94.34%,which was higher than 81.13%of routine coagulation test,and the difference was statistically significant(P<0.05).The transfusion volumes of platelet,blood plasma,cryoprecipitate and red blood cell transfusion in the observation group was less than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of blood transfusion-related complications between the two groups.Conclusion The application of thromboelastography in clinical blood transfusion guidance for patients with traumatic coagulopathy has good effect,which can effectively optimize the blood transfusion scheme,reduce the occurrence of blood transfusion-related complications,improve the safety of blood transfusion treatment,and has certain positive significance for guiding clinical early blood transfusion treatment.
