云南地区不孕不育夫妇生殖道支原体感染情况对比及差异分析

摘　　要：目的比较云南地区不孕不育夫妇生殖道支原体感染情况及药敏结果,分析夫妇差异感染的可能原因。方法回顾性分析2019年7月至2020年12月云南地区2211对不孕不育夫妇的临床资料,对生殖道解脲支原体(Uu)和人型支原体(Mh)的感染率、药敏结果进行统计,比较夫妇感染情况及药敏结果,并分析差异感染的原因。结果云南地区不育男性、不孕女性生殖道支原体感染率分别为48.21%、44.10%;Uu单一感染、Mh单一感染、Uu+Mh混合感染夫妇对交沙霉素敏感率均高于80%;夫妇感染生殖道支原体的一致率为49.31%,差异率为50.69%;云南地区感染Uu、Mh的不孕不育夫妇中,夫妇一致感染Uu、Mh的比例分别为27.76%、10.75%。结论云南地区不孕不育夫妇生殖道支原体感染率高,男性感染率较女性高;若处于云南省支原体药敏试验受限地区,对有治疗指征的支原体感染患者可首先考虑予以交沙霉素抗感染治疗;云南地区不孕不育夫妇感染生殖道支原体的差异率高,可能与Uu、Mh为机会感染病原体有关,Uu可能较Mh传染性更高。Objective To compare the mycoplasma infection in genital tract and drug sensitivity of infertile couples in Yunnan,and analyze the possible causes of differential infection.Methods The clinical data of 2,211 infertile couples in Yunnan from July 2019 to December 2020 were retrospectively analyzed.The infection rate and drug sensitivity results of ureaplasma urealyticum(Uu)and mycoplasma hominis(Mh)in the genital tract were statistically analyzed,and the infection and drug sensitivity results of the couples were compared,and the causes of differential infection were analyzed.Results The infection rates of mycoplasma in the genital tract of infertile men and women in Yunnan were 48.21%and 44.10%,respectively.The josamycin susceptibility rate of couples with Uu single infection,Mh single infection and Uu+Mh mixed infection was higher than 80%.The concordance rate of genital tract mycoplasma infection was 49.31%,and the difference rate was 50.69%.Among the infertile couples infected with Uu and Mh in Yunnan,the proportion of the couples infected with Uu and Mh were 27.76%and 10.75%respectively.Conclusions The infection rate of genital mycoplasma in infertile couples in Yunnan is high,and the infection rate of male is higher than the female.If the drug sensitivity test of mycoplasma is limited in Yunnan province,the first consideration for mycoplasma infection patients with treatment guidelines is to give josamycin anti-infection treatment.The difference rate of genital mycoplasma infection among infertile couples in Yunnan is high,which may be Uu and Mh as opportunistic pathogens,and Uu may be more infectious than Mh.
