摘　　要：目的 研究艾拉光动力与微波联合外用药物治疗对尖锐湿疣(CA)患者免疫功能及预后的影响。方法 选取2019年1月至2020年6月徐州医科大学附属医院收治的110例CA患者,采用随机数字表法分为对照组(n=55)和观察组(n=55)。对照组给予微波联合干扰素凝胶外用治疗,观察组在对照组基础上使用艾拉光动力联合治疗。评估并比较两组临床疗效;采用性功能评分表和症状体征评分表评估两组治疗前后性功能和症状体征变化情况;采用酶联免疫吸附试验法检测血清白介素-4(IL-4)、白介素-10(IL-10)和干扰素-γ(INF-γ)水平;采用流式细胞仪检测CD4~+、CD8~+、辅助T细胞(Th)1及Th2含量,并计算CD4~+/CD8~+和Th1/Th2;记录两组治疗期间不良反应发生情况和治疗后6个月的复发率。结果 观察组治疗总有效率为92.73%,明显高于对照组的78.18%(P<0.05);治疗后,观察组性功能评分高于对照组,症状体征评分低于对照组(P<0.05);观察组CD4~+、CD4~+/CD8~+及Th1/Th2水平高于对照组(P<0.05);观察组IL-4、IL-10及IFN-γ水平均明显高于对照组(P<0.05);观察组治疗期间不良反应总发生率为23.63%,高于对照组的16.36%,但差异无统计学意义(P>0.05);观察组复发率为11.54%,低于对照组的27.78%(P<0.05)。结论 艾拉光动力与微波联合外用干扰素凝胶治疗可提高CA患者的临床疗效,改善其症状体征,促进免疫功能恢复,同时降低复发率,改善患者预后。Objective To study the effects of Ella photodynamic therapy and microwave combined with drugs for external use on immune function and prognosis in patients with condyloma acuminatum(CA).Methods A total of 110 patients with CA admitted to Affiliated Hospital of Xuzhou Medical University between January 2019 and June 2020 were enrolled,and were divided into control group(n=55) and observation group(n=55) by random number table method.The control group was treated with microwave and topical interferon gel,while observation group was additionally treated with Ella photodynamic therapy.The clinical curative effect in the two groups was evaluated and compared.The changes of sexual function and symptoms in both groups before and after treatment were evaluated by sexual function scale and symptom sign scale.The levels of serum interleukin-4(IL-4),interleukin-10(IL-10) and interferon-γ(INF-γ) were detected by enzyme-linked immunosorbent assay.The levels of CD4^(+),CD8^(+),helper T cells(Th) 1 and Th2 were detected by flow cytometry.CD4^(+)/CD8^(+) and Th1/Th2 were calculated.The occurrence of adverse reactions during treatment and recurrence rates at 6 months after treatment in both groups were recorded.Results The total response rate of treatment in observation group was significantly higher than that in control group(92.73% vs.78.18%,P0.05),and recurrence rate was lower than that in control group(11.54% vs.27.78%,P<0.05).Conclusions Ella photodynamic therapy and microwave combined with topical interferon gel can improve clinical curative effect on CA patients,improve symptoms and signs,promote the recovery of immune function,reduce recurrence rate and improve prognosis.
