生殖器单纯疱疹病毒2型感染患者Th1/Th2及相关细胞因子表达与预后复发的相关性研究

摘　　要：目的 研究生殖器单纯疱疹病毒2型(HSV-Ⅱ)感染患者辅助T细胞(Th)1/Th2及相关细胞因子表达与预后复发的相关性。方法 选取2020年8月至2021年1月深圳市龙岗区第三人民医院收治的70例因HSV-Ⅱ感染的生殖器疱疹(GH)患者,均行临床规范化治疗。随访6个月,根据临床诊断结果将患者分为复发组、未复发组,检测患者皮损部位血液Th1、Th2及白介素(IL)-2、IL-10水平,Logistic回归分析法明确影响HSV-Ⅱ感染GH患者预后复发的危险因素,Pearson相关性分析法评估Th1/Th2及相关细胞因子与HSV-Ⅱ感染GH患者预后复发的相关性。结果 复发组24例,未复发组46例。与未复发组比较,复发组已婚率、合并其他性病率均较高,Th1/Th2、IL-2/IL-10均较低,差异具有统计学意义(P<0.05)。Logistic回归分析结果显示,已婚、合并其他性病、Th1/Th2、IL-2/IL-10是影响HSV-Ⅱ感染GH患者预后复发的危险因素。Pearson相关性分析结果显示,Th1/Th2、IL-2/IL-10与HSV-Ⅱ感染GH患者预后复发均呈负相关(r=-0.614、-0.577,P<0.001)。结论 HSV-Ⅱ感染GH复发患者Th1/Th2、IL-2/IL-10均较低,Th1/Th2失衡可能在HSV-Ⅱ感染GH患者预后复发机制中具有重要作用。Objective To study the relationship between the expression of T helper cell(Th)1/Th2 and related cytokines and the prognosis of relapse in patients with genital herpes simplex virus type 2(HSV-Ⅱ) infection.Methods 70 patients with genital herpes(GH) infected by HSV-Ⅱ who were admitted to Shenzhen Longgang District Third People′s Hospital from August 2020 to January 2021,were selected and treated with standardized clinical treatment.Follow-up for 6 months,according to the clinical diagnosis results,the patients were divided into recurrence group and non-recurrence group,detect the blood Th1,Th2,interleukin(IL)-2,and IL-10 levels in the patient′s skin lesions.Logistic regression analysis was used to identify the risk factors that affect the prognosis of HSV-Ⅱ infected GH patients.Pearson correlation analysis was used to evaluate the correlation between Th1/Th2 and related cytokines and the prognosis and recurrence of HSV-Ⅱ infected GH patients.Results There were 24 cases in the recurrence group and 46 cases in the non-recurrence group,compared with the non-recurrence group,the recurrence group had higher rates of marriage,and other sexual transmission dieases(STDs),and lower Th1/Th2,IL-2/IL-10,and significant differences between the groups(P<0.05).Logistic regression analysis showed that married,combined with other venereal diseases,Th1/Th2,IL-2/IL-10 are risk factors that affect the prognosis of HSV-Ⅱ infected GH patients.Pearson correlation analysis showed that Th1/Th2,IL-2/IL-10 were negatively correlated with the prognosis and recurrence of HSV-Ⅱ infected GH patients(r=-0.614,-0.577;P<0.001).Conclusions The Th1/Th2 and IL-2/IL-10 of patients with GH recurrence infected by HSV-Ⅱ are both low,Th1/Th2 cell imbalance may play an important role in the prognosis and recurrence mechanism of patients with GH infected by HSV-Ⅱ.
