C-TIRADS、ACR-TIRADS、K-TIRADS诊断甲状腺结节的对比研究

摘　　要：目的通过与ACR-TIRADS和K-TIRADS进行对比分析,探讨C-TIRADS的诊断价值。方法选取并分析1014例甲状腺结节的超声特征,使用三种TIRADS进行分类。以病理结果为金标准构建ROC曲线,比较三种TIRADS的诊断效能,以及不必要活检率。将结节分为>10 mm组和≤10 mm组,比较两组的诊断效能。结果C-TIRADS、ACR-TIRADS、K-TIRADS的AUC分别为0.939、0.924、0.921,C-TIRADS高于其它两组(P<0.01)。C-TIRADS的灵敏性最高(P<0.001),ACR-TIRADS的特异性最高(P<0.05)。C-TIRADS(25.70%)、ACR-TIRADS(25.91%)的不必要活检率低于K-TIRADS(45.22%)(P<0.001)。C-TIRADS和K-TIRADS的AUC在大小两组之间的差异无统计学意义(P>0.05),ACR-TIRADS差异有统计学意义(P<0.05)。结论C-TIRADS的诊断效能高于ACR-TIRADS和K-TIRADS。C-TIRADS具有较低的不必要活检率。三种指南对两组患者均具有较高的诊断效能。Objective The diagnostic value of C-TIRADS was discussed through comparative analysis with ACR-TIRADS and K-TIRADS.Methods Ultrasound features of 1014 thyroid nodules were retrospectively analyzed and classified using three TIRADS.ROC curve was constructed with pathological results as the gold standard to compare the diagnostic efficiency and unnecessary biopsy rate of three TIRADS.The nodules were divided into>10 mm group and≤10 mm group to compare the diagnostic efficacy of the two groups.Results The AUC of C-TIRADS,ACR-TIRADS and K-TIRADS were 0.939,0.924 and 0.921,respectively.The AUC of C-TIRADS was higher than that of the other two groups(P<0.01).The sensitivity of C-TIRADS was the highest(P<0.001),and the specificity of ACR-TIRADS was the highest(P<0.05).The unnecessary biopsy rate of C-TIRADS(25.70%)or ACR-TIRADS(25.91%)was lower than that of K-TIRADS(45.22%)(P<0.001).The AUC of C-TIRADS and K-TIRADS showed no statistical significance between big group and small group(P>0.05),while,that of ACR-TIRADS showed statistical significance(P<0.05).Conclusion The diagnostic efficiency of C-TIRADS is higher than that of ACR-TIRADS and K-TIRADS.C-TIRADS has low unnecessary biopsy rate.The three guidelines have high diagnostic efficiency in big group and small group.
