摘  要:目的通过对肺癌患者行双源CT双能量多参数成像,探讨其在肺癌诊断中的应用价值。方法选取我院经病理证实的肺癌患者66例,分为腺癌组30例,鳞癌组24例,小细胞肺癌组12例,于治疗前行胸部双源双能量CT平扫及动静脉双期增强扫描。分别测量各组肺癌原发病灶各期参数指标,包括碘浓度(IC)、标准化碘浓度(NIC),绘制能谱曲线、40~70 keV能谱曲线斜率(K_(40~70 keV))。比较三组各期参数指标并进行统计,评估双源CT双能量成像定量参数在鉴别肺癌亚型的价值。结果平扫时鳞癌组与小细胞肺癌组、腺癌与小细胞肺癌组能谱曲线斜率K_(40~70 keV)差异均有统计学意义(P均<0.05),鳞癌组与腺癌组能谱曲线斜率K_(40~70 keV)差异无统计学意义(P>0.05);动、静脉期3组间能谱曲线斜率K_(40~70 keV)、IC、NIC差异有统计学意义(P<0.05)。结论双源CT双能量成像对肺癌病理类型诊断具有一定的价值。Objective To investigate the diagnostic value of dual-source CT dual-energy multi-parameter imaging in lung cancer patients.Methods 66 patients with lung cancer confirmed by pathology in our hospital were collected and divided into 3 groups,including adenocarcinoma group(n=30),squamous cell carcinoma group(n=24)and small cell lung cancer group(n=12).Parameters at each stage of primary lung cancer lesions in each group were measured,including iodine concentration(IC)and standardized iodine concentration(NIC),and energy spectrum curve and slope of energy spectrum curve(K_(40~70 keV))were plotted.Parameters of the three groups were compared and statistically analyzed to evaluate the value of quantitative parameters of dual-source CT dual-energy imaging in differentiating lung cancer subtypes.Results There were statistically significant differences in energy spectrum slope K_(40~70 keV)between squamous cell carcinoma group and small cell lung cancer group,adenocarcinoma group and small cell lung cancer group(ALL P<0.05),but there were no statistically significant differences between squamous cell carcinoma group and adenocarcinoma group(P>0.05).The slope of energy spectrum curves K_(40~70 keV),IC and NIC between the three groups in arteriovenous and venous phases had statistical significance(ALL P<0.05).Conclusion Dual-source CT dual-energy imaging has certain value in the diagnosis of pathological types of lung cancer.
