胃肠道EP的多模态CT特征差异性分析

摘　　要：目的探讨胃肠道异位胰腺(EP)多期扫描的CT表现、临床资料与正常胰腺、不同性别之间的关系。方法选取24例经病理证实的EP患者的临床资料、CT图像,收集并分析病灶的位置、形状、长径、短径、长径/短径、生长方式、强化方式、囊变、钙化、“导管征”、周围淋巴结、各期EP及正常胰腺的CT值。结果EP多为单发、卵圆形、腔内生长、均匀强化的软组织密度肿块,少见“导管征”,无钙化、囊变、周围淋巴结增大,动脉期EP与正常胰腺的CT值差异有统计学意义(P<0.05),平扫及门静脉期EP与正常胰腺的CT值差异无统计学意义(P>0.05),EP与正常胰腺的动脉期强化相对值、门静脉期强化相对值之间差异有统计学意义(P<0.05);不同性别患者的年龄、EP的长径、短径差异无统计学意义(P>0.05),不同性别患者的EP平扫CT值、动脉期CT值、门静脉期CT值、动脉期强化相对值、门静脉期强化相对值之间差异无统计学意义(P>0.05)。结论通过对比EP与正常胰腺、不同性别患者EP的影像表现,对于胃肠道EP的诊断具有一定的意义。Objective To investigate the relationship of CT findings and clinical data of gastrointestinal ectopic pancreas between normal pancreas and differences in phase.Methods The clinical data and CT images of 24 ectopic pancreas patients confirmed by pathology were collected and analyzed,including the location,shape,long diameter,short diameter,long diameter/short diameter,growth mode,enhancement mode,cystic degeneration,calcification,ductal sign,surrounding lymph nodes,CT values of ectopic pancreas,and normal pancreas at each stage.Results Most of the ectopic pancreases were single,oval,intraluminal growth,uniformly enhanced soft tissue density mass,rare ductal sign,without no cystic changes,surrounding lymph node enlargement,or calcification.The difference between the CT values of the ectopic pancreas and normal pancreas in the arterial phase was statistically significant(P<0.05).There was no significant difference in the CT value between ectopic pancreas and normal pancreas in the plain scan and portal vein phase(P>0.05).There were significant differences in the relative values of arterial phase enhancement and portal venous phase enhancement between ectopic pancreas and normal pancreas(P<0.05).There was no significant difference in the age,long diameter,short diameter among different sexes(P>0.05).There was no significant difference in plain scan,arterial phase and portal venous phase CT value of ectopic pancreas,arterial phase enhancement relative value or portal vein phase enhancement relative value of ectopic pancreas among different sexes(P>0.05).Conclusion Comparing the imaging findings of ectopic pancreas with normal pancreas and patients of different sexes has certain significance for the diagnosis of gastrointestinal ectopic pancreas.
