摘  要:目的探讨肩胛盂矢状面倾斜角(GSIA)与肩袖慢性损伤的MRI相关性。方法选取2018年8月~2020年6月符合肩袖慢性损伤诊断标准的患者145例。根据MRI表现,将肩袖肌腱损伤分为4组,A组单纯冈上肌腱损伤,B组冈上肌腱合并冈下/小圆肌腱损伤,C组冈上肌腱合并肩胛下肌腱损伤,D组全肩袖肌腱损伤。在PACS系统测量GSIA、临界肩角(CSA)、肩肱间距(AHD)、喙肱间距(CHD)等肩关节各骨间位置关系参数。分析各参数与不同类型肩袖损伤的相关性。结果四组的GSIA、AHD、CHD差异有统计学意义(P<0.05),A组GSIA为(34.9±2.2)°,B组(46.9±4.7)°,C组(30.2±2.6)°,D组(41.4±3.6)°;CSA差异无统计学意义(P=0.851)。Logistic回归分析显示GSIA和CHD为肩袖损伤部位的影响因素。ROC曲线确定B组的截断值为43.4°。结论肩胛盂矢状面倾斜角与肩袖慢性损伤之间存在相关性,GSIA对预测肩袖损伤有一定参考价值。Objective To study the association between glenoid sagittal inclination angle(GSIA)and chronic rotator cuff tear.Methods 145 patients who met the diagnostic criteria of chronic rotator cuff tear were collected from August 2018 to June 2020.According to MRI,the shoulders were classified into 4 groups(Group A:isolated supraspinatus tendon tear;Group B:both the supraspinatus and infraspinatus tendons tears;Group C:both the supraspinatus and subscapularis tendons tears;and Group D:global rotator cuff tears).The PACS system was used for measuring the parameters such as GSIA,critical shoulder angle(CSA),acrominal-humeral distance(AHD),coracoid-humeral distance(CHD),then correlation between these parameters and rotator cuff tear was analyzed.Results There were significant differences in GSIA,AHD,CHD(P<0.05)and no differences in CSA(P=0.851)among the 4 groups.GSIA of 4 groups was(34.9±2.2)°,(46.9±4.7)°,(30.2±2.6)°,(41.4±3.6)°respectively.Logistic regression analysis indicated GSIA and CHD were the affecting factors of rotator cuff tear.ROC curve determined the cut-off value of group B was 43.4°.Conclusion GSIA is related with chronic rotator cuff tear,which is a predictable mark of rotator cuff tear.
