摘 要:埃可病毒11型(Echovirus 11,E11)属于B组肠道病毒,是最常见的人类肠道病毒(Enterovirus,EV)之一,是引起无菌性脑膜炎,新生儿感染的重要病原体之一。为探讨广州市生活污水监测中E11分离株循环动态变化及其基因组特征,本研究对2009-2021年广州市生活污水EV监测获得的E11分离株进行VP1区基因扩增及测序,结合2019年广东省发生的两起E11新生儿感染事件及部分散发病例中获得的E11分离株,与全球E11参考株进行序列相似性分析和系统发生学分析。结果显示,2009-2021年生活污水EV监测中E11分离株的核苷酸及氨基酸相似性分别为72.8%~99.3%和84.5%~99.6%。监测发现,广州市2018-2019年在污水中检出大量E11,与两起新生儿感染事件及散发病例的分离株核苷酸相似性高达96.2%~99.2%,但序列间仍存在差异,两起新生儿感染及散发病例感染的毒株为不同来源。通过系统进化树分析发现,2018-2019年污水E11分离株及两起新生儿感染与散发病例E11亲缘关系较近,分子分型结果均为E11 D5基因型。2017年及以前的污水分离株以A1基因型为主,但也有C3和D5基因型,2018年开始则以D5基因型为主,与同时期EV感染病例基因型结果一致。而在2021年污水中则发现了E11 E基因型,但广州暂未有该基因型的感染病例报道,仍需持续进行病例和环境监测。结果表明,生活污水监测能够及时反映EV在环境和人群中的循环动态变化,对EV引起的相关疾病起到预测预警作用。Echovirus 11(E11),classified into enterovirus(EV)group B,is one of the most commonly isolated human EVs.Most lethal EV infections and aseptic meningitis are caused by E11.We planned to analyze the genetic characterization of E11 from samples of domestic sewage in Guangzhou City(China).The VP1 coding region of the strains were amplified.Phylogenetic analyses on the VP1 sequences from these isolates,strains isolated from two E11 neonatal infection events,and some sporadic cases in Guangdong Province in 2019 and others published in GenBank were conducted.From 2009 to 2021,several E11 strains were isolated from samples of domestic sewage in Guangzhou City in Guangdong Province,China.Among these isolates,nucleotide(nt)identities were 72.8%–99.3%,whereas amino acid(aa)identities were 84.5%–99.6%.The nt identities were 96.2%–99.2%between the strains isolated from sewage in 2018–2019 and the strains isolated from two neonatal infection events as well as some sporadic cases in Guangdong Province in 2019,China.The strains mentioned above had the D5 genotype.However,there were differences between the sequences,and the two events and sporadic cases were infected with strains from different sources.From 2009to 2017,the dominant transmission chain of E11 was the A1 genotype,but there were also C3 and D5genotypes.Since 2018,the dominant transmission chain of E11 was the D5 genotype,which was consistent with the genotyping results of the EV infection cases during the same period.Genotype E was found in isolates of domestic sewage in 2021,but no case of infection with this genotype was reported in Guangzhou,China.Continued surveillance of cases and sewage is required.In conclusion,timely surveillance of domestic sewage reflects the circulation of human EVs,which has practical importance for prediction and early warning of human EV-related diseases.
