摘　　要：目的 探讨某三级肿瘤专科医院子宫颈病变患者2018—2020年住院费用的构成及其影响因素,为优化医疗资源配置,改善医疗服务行为,降低疾病经济负担提供切实依据。方法 收集确诊为子宫颈病变患者的住院资料3 136例,费用使用贴现率调整,单因素分析采用非参数检验,多因素分析采用多元线性回归分析,以P<0.05为差异有统计学意义。结果 2018—2020年宫颈病变患者例均住院总费用为55 699.71元,宫颈上皮内瘤变患者例均住院总费用为17 812.90元,年龄、医保类型、临床分期、治疗方式、住院总天数在多元线性回归分析中具有统计学差异(P<0.001)。结论 宫颈上皮内瘤变患者例均住院总费用明显低于较其他期患者,例均医疗费用呈逐年下降趋势,但例均耗材费却逐年增加。年龄、医保类型、临床分期、治疗方式、住院总天数会影响住院医疗费用。Objective To explore the composition and influencing factors of hospitalization expenses for cervical lesions in a tertiary oncology hospital from 2018 to 2020,so as to provide a practical basis for optimizing the allocation of medical resources,improving medical service behavior,and reducing the economic burden of disease.Methods The hospitalization data of 3 136 patients diagnosed with cervical lesions were collected,the cost was adjusted by discount rate,nonparametric test was used for univariate analysis,and multiple linear regression analysis was used for multivariate analysis,P<0.05 was considered statistically significant.Results From 2018 to 2020,the total hospitalization cost per case of cervical lesions was 55 699.71 yuan,and the average total hospitalization cost of CIN patients was 17 812.90 yuan.The age,medical insurance type,clinical stage,treatment method,and total hospitalization days were statistically significant in the multiple linear regression analysis.difference(P<0.001).Conclusion The total cost of hospitalization per case of CIN patients is significantly lower than that of patients in other stages.Age,medical insurance type,clinical stage,treatment method,and total hospitalization days will affect hospitalization medical expenses.
