柴芍六君子汤防治原发性肝癌肝动脉化疗栓塞术后栓塞综合征临床研究

摘　　要：目的:观察柴芍六君子汤对原发性肝癌肝动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)术后栓塞综合征的防治作用。方法:将118例经TACE治疗的原发性肝癌患者按照随机数字表法分为观察组和对照组,每组各59例。对照组术后给予常规对症治疗,观察组在对照组治疗的基础上加服柴芍六君子汤。比较两组患者的临床疗效、栓塞综合征发生率及持续时间,观察两组患者治疗前后肝功能、甲胎蛋白(alpha-fetoprotein,AFP)及卡氏评分量表(karnofsky performance status,KPS)评分变化情况。结果:观察组有效率为89.83%,对照组有效率为67.80%,两组患者有效率比较,差异具有统计学意义(P<0.05)。观察组术后发热、肝区疼痛、呕吐发生率及持续时间低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后各项肝功能指标低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后AFP水平低于本组治疗前,差异具有统计学意义(P<0.05),观察组治疗后AFP水平低于对照组,但差异无统计学意义(P>0.05)。两组患者治疗后KPS积分高于本组治疗前,且治疗后观察组高于对照组,差异具有统计学意义(P<0.05)。结论:柴芍六君子汤对原发性肝癌TACE术后栓塞综合征有较好的防治作用,可减轻化疗后的不良反应,改善肝功能,提高患者生存质量。Objective:To observe the preventive and therapeutic effects of Bupleurum and Peony Six Gentlemen Decoction on embolism syndrome after transcatheter arterial chemoembolization(TACE) for primary liver caner.Methods:A total of 118 patients with primary liver cancer treated by TACE were randomly divided into the observation group and the control group,with 59 cases in each group.The patients in the control group were given routine symptomatic treatment after operation,and the observation group was given Bupleurum and Peony Six Gentlemen Decoction on the basis of the treatment in the control group.The clinical efficacy,incidence and duration of embolism syndrome were compared between the two groups,and the changes of liver function and alpha fetoprotein(AFP) level before and after treatment were detected.Results:The effective rate of the observation group was 89.83%,and that of the control group was 67.80%.The difference between the two groups was statistically significant(P0.05).The KPS score of the two groups after treatment was higher than that of the group before treatment,and the difference between the two groups was statistically significant(P<0.05).Conclusion:Bupleurum and Peony Six Gentlemen Decoction has a good preventive and therapeutic effect on embolism syndrome after TACE for primary liver cancer.It can reduce adverse reactions after chemotherapy,improve liver function and improve the quality of life of patients.
