得气针刺治疗膝关节骨性关节炎临床研究

摘　　要：目的:观察得气针刺对膝关节骨性关节炎慢性疼痛的持续性效应。方法:将138例膝关节骨性关节炎患者按照随机数字表法分为针刺得气组、针刺不得气组和等待治疗组,每组各46例。针刺得气组予以正常针刺后行得气治疗,针刺不得气组予以浅刺破皮不得气治疗,等待治疗组不予以治疗。比较三组患者临床疗效及治疗前后西安大略麦克马斯特大学骨关节炎指数可视化量表评分(Western ontario and mcmaster university osteoarthriti,WOMAC)、视觉模拟评分(visual analogue scale,VAS)变化情况。结果:针刺得气组、针刺不得气组治疗后WOMAC评分、VAS评分低于本组,治疗后针刺得气组低于针刺不得气组和等待治疗组,针刺得气组改善值高于针刺不得气组与等待治疗组,差异均有统计学意义(P<0.05)。针刺得气组入组8周、12周及24周WOMAC评分改善值高于针刺不得气组、等待治疗组,差异均有统计学意义(P<0.05)。针刺得气组有效率为93.2%,针刺不得气组有效率为61.9%,等待治疗组有效率为15.9%,针刺得气组有效率高于针刺不得气组和等待治疗组,差异有统计学意义(P<0.05)。结论:得气针刺治疗膝关节骨性关节炎,可改善患者疼痛程度及运动功能障碍,具有一定的长期持续效应。Objective:To observe the lasting effect of acupuncture obtaining qi on chronic pain of knee osteoarthritis.Methods:A total of 138 patients were divided into three groups according to the method of random number table:the group of acupuncture obtaining qi,the group of acupuncture without obtaining qi,and the group of waiting for treatment,with 46 cases in each group.The group of acupuncture obtaining qi was treated with acupuncture obtaining qi after normal acupuncture;the group of acupuncture without obtaining qi was treated by shallow puncture,and the waiting group was not treated.The clinical efficacies of the three groups were compared,and the changes of Western Ontario and McMaster University Osteoarthritis Index(WOMAC) and visual analogue scale(VAS) before and after treatment were compared.Results:After treatment,the WOMAC score and VAS score of the group of acupuncture obtaining qi,and the group of acupuncture without obtaining qi before treatment.The above indexes of the group of acupuncture obtaining qi were lower than those of the group of acupuncture without obtaining qi and the group of waiting for treatment.The improvement value of the group of acupuncture obtaining qi was higher than that of the group of acupuncture without obtaining qi and the group of waiting for treatment,and all the differences were statistically significant(P<0.05).After 8 weeks,12 weeks and 24 weeks of treatment,the WOMAC improvement value of the group of acupuncture obtaining qi was all higher than that of the group of acupuncture without obtaining qi and the group of waiting for treatment,and all the differences were statistically significant(P<0.05).The effective rate of the group of acupuncture obtaining qi was 93.2%,which was higher than that of the group of acupuncture without obtaining qi and the group of waiting for treatment was 61.9% and 15.9% respectively,and all the differences were statistically significant(P<0.05).Conclusion:The treatment of knee osteoarthritis with acupuncture obtaining qi can improve the pain de
