肾精亏虚型颈动脉粥样硬化患者载脂蛋白E基因多态性的临床分析

摘　　要：目的观察肾精亏虚型颈动脉粥样硬化患者载脂蛋白E(APOE)基因多态性特征。方法将70例颈动脉粥样硬化患者根据中医辨证分型分为肾精亏虚型与非肾精亏虚型,各35例。采集2组患者基本信息、检测APOE基因多态性,并进行比较。结果与非肾精亏虚型组比较,肾精亏虚型组颈动脉粥样硬化患者合并冠心病、脑梗死比例更高(P<0.05)。2组间APOE基因多态性比较,肾精亏虚型组颈动脉粥样硬化患者E4亚型(ε3/ε4)较非肾精亏虚型颈动脉粥样硬化组所占比例更高,而E3亚型(ε3/ε3)所占比例更低(P<0.05)。结论肾精亏虚型颈动脉粥样硬化患者常见E4亚型(ε3/ε4),临床可将血清APOE基因多态性检测作为颈动脉粥样硬化中医辨证及治疗的一项参考指标。Objective To investigate the polymorphism characteristics of apolipoprotein E(APOE)gene in patients with carotid atherosclerosis of kidney essence deficiency type.Methods According to traditional Chinese medicine syndrome differentiation,70 patients with carotid atherosclerosis were divided into kidney essence deficiency type group and non-kidney essence deficiency type group,35 cases in each group.The basic information and APOE gene polymorphism were collected and compared between the two groups.Results Compared with non-kidney essence deficiency type group,the proportion of carotid atherosclerosis patients complicated with coronary heart disease and cerebral infarction was higher in kidney essence deficiency type group(P<0.05).The comparison of APOE gene polymorphism between the two groups showed that the proportion of E4 subtype(ε3/ε4)in carotid atherosclerosis patients with kidney essence deficiency type group was higher than that of carotid atherosclerosis patients without kidney essence deficiency type group,while the proportion of E3 subtype(ε3/ε3)was lower(P<0.05).Conclusion E4 subtype(ε3/ε4)is common in carotid atherosclerosis with kidney essence deficiency type.The detection of serum APOE gene polymorphism can be used as a reference index for traditional Chinese medicine syndrome differentiation and treatment of carotid atherosclerosis.
