规律锻炼和呼吸训练结合饮食干预对慢性阻塞性肺疾病伴糖尿病的疗效

摘　　要：目的分析饮食联合规律体育锻炼结合呼吸训练在慢性阻塞性肺疾病合并糖尿病患者中的应用效果,探讨该类患者营养状态、血气指标及肺差值、血糖等指标的影响。方法选择2019年11月至2021年11月收治的60例慢性阻塞性肺疾病合并糖尿病患者,采取随机数表法分为参照组和观察组,每组30例。患者均接受常规干预,观察组采取饮食联合规律体育锻炼结合呼吸训练模式。连续干预3个月,观察2组营养状态、血气指标及肺差值。结果观察组糖化血红蛋白、欧洲营养风险筛查量表评分均低于参照组(P<0.05);观察组pH值、二氧化碳分压高于参照组(P<0.05);观察组氧分压指标低于参照组(P<0.05);观察组1 s用力呼气容积、用力肺活量比值及呼吸峰值流速均高于参照组(P<0.05);观察组空腹血糖、餐后2 h血糖水平低于参照组(P<0.05)。结论饮食联合规律体育锻炼与呼吸训练治疗对慢性阻塞性肺疾病合并糖尿病患者营养状态具有积极作用,能改善其气血指标,促使患者肺部功能得以恢复,稳定血糖,有利于其预后。Objective To investigate the therapeutic effects of diet intervention combined with regular physical exercise and breathing training on chronic obstructive pneumonia complicated by diabetes mellitus,and to explore their effects on nutritional status,blood gas indicators,lung difference value and blood sugar indexes of patients.Methods Sixty patients with chronic obstructive pneumonia complicated by diabetes mellitus who were treated in our hospital from November 2019 to November 2021 were enrolled in the study,who were randomly divided into control group and observation group,with 30 cases in each group.The patients in both groups were treated by conventional intervention,on this basis,the patients in observation group were treated by diet intervention combined with regular physical exercise and breathing training,for 3 months.The nutritional status,blood gas indexes and lung difference value were observed and compared between the two groups.Results After 3-month intervention,the scores of glycosylated hemoglobin and European Nutrition Risk Screening Scale in observation groups were significanatly lower than those in control group(P<0.05),however,the pH value and carbon dioxide partial pressure were significanatly higher than those in control group(P<0.05).Moreover the oxygen partial pressure levels in observation groups were significanatly lower than those in control group(P<0.05),however,the forced expiratory volume per second,forced vital capacity ratio and peak respiratory flow rate were significanatly higher than those in control group(P<0.05).In addition the levels of fasting blood glucose,postpradial 2h blood glucose in observation groups were significanatly lower than those in control group(P<0.05).Conclusion The combination application of diet intervention combined with regular physical exercise and breathing training has a positive role in improving the nutritional status of patients with chronic obstructive pneumonia complicated by diabetes mellitus,which can improve the qi and blood indexes,promote th
