摘 要:目的应用临床及蛋白质组学结合的方法观察补肾化痰方治疗多囊卵巢综合征(polycystic ovarian syndrome,PCOS)痰湿证的疗效。方法以鹿特丹诊断标准纳入PCOS痰湿证患者30例,应用补肾化痰方治疗3个月,采集治疗前后临床及血清样本,进行性激素、糖脂代谢水平检测,并选取12例血清样本采用相对和绝对定量同位素标记(iTRAQ)联合液相色谱串联质谱(LC-MS/MS)技术筛选治疗前后血清差异表达蛋白。结果补肾化痰方治疗后患者空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、载脂蛋白B/A1(ApoB/A1)显著下降,SHBG较治疗前升高;经质谱分析PCOS痰湿证组与健康对照组比较,共发现87个差异蛋白,上调蛋白10个,下调蛋白77个。补肾化痰方组与PCOS痰湿证组比较,共发现63个差异蛋白,上调蛋白25个,下调蛋白38个。在PCOS痰湿证组表达上调或下调,经补肾化痰方干预后,明显回调的蛋白有7个。补肾化痰方干预后主要富集的KEGG通路包括补体和凝血级联、Fc-γ-R介导的吞噬作用和Jak-STAT信号通路。结论补肾化痰方治疗PCOS痰湿证的作用机制可能与调控炎症反应、糖脂代谢、抗氧化反应、补体活化调节等病理环节相关。Objective To observe the therapeutic effect of Bushen Huatan Decoction(补肾化痰方)on polysystic ovary syndrome(PCOS)of phlegm-dampness syndrome of by clinical and proteomic methods.Methods Thirty PCOS of phlegm-dampness syndrome of were enrolled according to the Rotterdam diagnostic criteria.They were treated with Bushen Huatan Decoction for 3 months.The clinical and serum samples were collected before and after treatment to detect the levels of sex hormone,glucose and lipid metabolism.Twelve serum samples were selected to screen the differentially expressed proteins before and after treatment by relative and absolute quantitative isotope labeling(iTRAQ)combined with ultra high liquid chromatography tandem mass spectrometry(LC-MS/MS).Results The levels of fast insuline(FINS),homeostatic model assessment of insulin resistance(HOMA-IR),triglyceride(TG)and apolipoprotein B(apoB)/apolipoprotein A1(ApoA1)were decreased significantly after treatment with Bushen Huatan Decoction while sex hormone binding globulin(SHBG)increased.There were 87 proteins with significant differences(P<0.05)in phlegm-dampness type of PCOS group compared with those of the normal group,in which 10 proteins were up-regulated and 77 proteins were down-regulated.A total of 63 proteins with significant differences(P<0.05)inBushen Huatan Decoctiongroup compared with those of the phlegm-dampness type of PCOS group,in which 25 proteins were up-regulated and 38 proteins were down-regulated.After the intervention of Bushen Huatan Decoction,there were 7 proteins in phlegm-dampness type of PCOS group were obviously regulated.After the intervention of Bushen Huatan Decoction,the mainKEGG pathways included complements,coagulation cascade and FC-γ-R-mediated phagocytosis and Jak-STAT signaling pathway.Conclusion The mechanism of Bushen Huatan Decoction in the treatment of PCOS of phlegm-dampness syndrome may be related to the regulation of inflammatory reaction,glucose and lipid metabolism,antioxidant reaction and complement activation regulation.
