于天源治疗神经根型颈椎病的推拿辨证及治则治法

摘　　要：颈椎病分为颈型、神经根型、椎动脉型、脊髓型、交感型和混合型6种类型,其中神经根型颈椎病是常见的颈椎病类型之一,表现为颈项部肌肉僵硬、酸痛,活动受限,常出现上肢麻木疼痛波及至手指。临床应用推拿手法治疗疾病时,须提出与推拿临床实践相应的治则治法,即在临床辨证的前提下提出相应治疗原则,施用相应推拿治法。其中治则是治疗疾病时须遵循的法则,是用以指导治疗方法的总则;而治法是在治则的指导下制定的治疗疾病的具体方法,它从属于治则,通常是多项治则联合多项治法同时使用。该文结合临床,阐述了于天源教授对推拿手法治疗神经根型颈椎病在临床辨证及治则治法的运用经验,在临床治疗中于天源教授运用推拿手法治疗神经根型颈椎病遵循以伤科辨证为主的原则,总结主要治则治法包括:强则松之、塞则通之、凝则动之、乱则复之、收则散之,并与同学科系列教材治疗本病的治则治法进行对比分析,强调推拿治疗需辨证论治,且以伤科辨证为主。Cervical spondylosis can be divided into six types:cervical type,radicular type,vertebral artery type,myeloid type,sympathetic type and mixed type.Radicular type of cervical spondylosis is one of the most common types of cervical spondylosis,which is characterized by stiffness,pain and limited activity of neck muscles,and often numbness and pain of upper limbs affecting fingers.In the clinical application of Tuina,it is necessary to put forward the corresponding therapeutic principles and therapeutic methods corresponding to the clinical practice of Tuina,that is,to put forward the corresponding therapeutic principles and apply the corresponding therapeutic methods of Tuina on the premise of clinical syndrome differentiation.Among them,the therapeutic principle is the law that must be followed when treating the disease,and is the general principle that guides the treatment method.A treatment is a specific method to treat a disease under the guidance of a treatment principle.It is subordinate to a treatment principle,usually multiple treatment principles combined with multiple treatments are used simultaneously.Combined with the clinical practice,this paper expounds Professor YU Tianyuan's experience on clinical syndrome differentiation and treatment of cervical spondylotic radiculopathy by Tuina.In clinical treatment,Professor YU Tianyuan's use of Tuina in the treatment of cervical spondylotic radiculopathy follows the principle of syndrome differentiation of trauma,and summarizes the main treatment methods,including relaxing when strong,relaxing when blocking,moving when coagulating,recovering after disorder and dispersing when collecting.It is also compared with the treatment principle and treatment method of treating this disease with the series of teaching materials of the same department.It emphasizes that Tuina should be based on syndrome differentiation and the injury syndrome differentiation is the main method.
