复荣通脉胶囊治疗糖尿病高危足疗效观察

摘　　要：目的观察复荣通脉胶囊治疗气阴两虚、瘀阻经络证糖尿病高危足对患者多伦多神经病变评分(TCSS)及预后的影响。方法将2018年6月—2020年6月期间在该院治疗的120例糖尿病高危足患者作为观察对象,按照随机数字表法将其分为治疗组与对照组,每组各60例。对照组患者给予常规西药治疗,治疗组患者在此基础上联合使用复荣通脉胶囊治疗,两组患者均治疗24周。比较治疗前(T0)、治疗第4周(T1)、12周(T2)、24周(T3)两组患者中医症状评分(口渴喜饮、少气懒言、五心烦热、麻木刺痛)、神经受损程度(TCSS评分)、踝肱指数(ABI值)、足底峰值压力及神经传导速度。结果T0时,两组患者中医症状评分、TCSS评分、ABI值、足底峰值压力及神经传导速度比较差异无统计学意义(P>0.05),T0~T3时,两组患者各中医症状评分、TCSS评分及足底峰值压力均呈下降趋势,两组患者ABI值及神经传导速度均呈上升趋势,且T1~T3时,治疗组下降或上升趋势均优于低对照组(P<0.05)。结论复荣通脉胶囊治疗气阴两虚、瘀阻经络证糖尿病高危足,可有效改善患者临床症状,降低TCSS评分,减轻足底峰值压力,提高ABI水平及神经传导速度,改善预后。Objective To observe the effect of Furong Tongmai Capsule(复荣通脉胶囊)on Toronto Clinical Scoring System(TCSS)and prognosis of patients with high risk diabetic foot of Qi-Yin deficiency and blood stasis blocking meridian syndrome.Methods Totally 120 cases of patients with high risk diabetic foot treated in the hospital from June 2018 to June 2020 were taken as the observation subjects,and they were divided into the treatment group and the control group according to the random number table method,with 60 cases in each group.Patients in the control group were treated with conventional western medicine,and those in the treatment group were combined with Furong Tongmai Capsule on this basis,and patients in both groups were treated for 24 weeks.The TCM symptoms scores(thirst with liking for fluids,lack of energy,dysphoria in chest-palms-soles,numbness and tingling),TCSS score,ankle-brachial index(ABI value)and peak plantar pressure and nerve conduction velocity were compared between the two groups before treatment(T0)and on the 4^(th) week(T1),12^(th) week(T2)and 24^(th) week(T3)of the treatment.Results At T0,there were no significant differences in TCM symptoms scores,TCSS score,ABI value,peak plantar pressure or nerve conduction velocity between the two groups(P>0.05).At T0-T3,the TCM symptoms scores,TCSS score and peak plantar pressure of the two groups showed downward trends while the ABI value and nerve conduction velocity of the two groups showed upward trends,and at T1-T3,the downwards or upwards of the treatment group were better than those of the control group(P<0.05).Conclusion Furong Tongmai Capsule can effectively improve the clinical symptoms,lower the TCSS score,reduce the peak plantar pressure,increase the ABI level and nerve conduction velocity,and improve the prognosis.
