壮医莲花针拔罐逐瘀法对前交叉韧带重建术后本体感觉恢复的影响

摘　　要：目的观察壮医莲花针拔罐逐瘀法对前交叉韧带(ACL)重建术后本体感觉恢复的影响。方法选择2017年9月—2019年3月行关节镜下ACL重建术的患者52例作为研究对象。根据术后康复治疗方案的不同,将以上患者按照随机数字表法分为观察组、对照组各26例。其中对照组男16例,女10例,予股四头肌收缩等常规的康复训练法;观察组男17例,女9例,则予壮医莲花针拔罐逐瘀法作为术后康复治疗方案。分别于术前及术后第3、6、9个月比较两组患者的Lysholm膝关节功能评分、国际膝关节委员会主观评分表(IKDC)评分以及被动角度再生试验、被动活动察觉阀值,评判和了解膝关节的基本活动功能和本体感觉的恢复情况。结果比较分析两组患者术前Lysholm评分、IKDC评分、被动角度再生试验结果、被动活动察觉阀值结果,以及两组患者一般情况(性别、年龄、病程、断裂部位、损伤原因),均未见明显差异。所有的受试者术后均获得9个月以上的随访。术后第3、6、9个月,观察组的Lysholm评分及IKDC评分均高于对照组(P<0.05);术后第3、6、9个月,观察组被动活动察觉阀值均低于对照组(P<0.05);术后第3、6个月,观察组被动角度再生试验低于对照组(P<0.05)。结论关节镜下ACL重建术后患者,选用壮医莲花针拔罐逐瘀法作为康复治疗方案,能更好的提高膝关节的稳定性及活动功能,从而进一步促进了膝关节本体感觉的有效恢复。Objective To observe the effect of Zhuang medicine lotus needling and cupping method on proprioception recovery after anterior cruciate ligament(ACL)reconstruction.Methods Fifty-two patients undergoing ACL arthroscopic reconstruction from September 2017 to March 2019 in the hospital were selected as subjects.According to the difference of postoperative rehabilitation treatment,the above patients were randomly divided into the observation group and the control group,with 26 cases in each group.In the control group,there were 16 males and 10 females given conventional rehabilitation training such as quadriceps femoris contraction.In the observation group,there were 17 males and 9 females and the lotus needling and cupping method were added on the basis of the control group.The basic movement function of knee joint and the change of proprioception,the Lysholm knee function score,the International Knee Committee subjective rating scale(IKDC)score,passive angle regeneration test and passive activity detection threshold were compared before and after 3,6 and 9 months of the operation respectively.Results The preoperative Lysholm score,IKDC score,passive angle regeneration test results,passive activity detection threshold results,and general conditions(gender,age,course of disease,site of fracture,cause of injury)of the two groups were compared and analyzed.No significant differences were found between the two groups.All subjects were followed up for more than 9 months after operation.The Lysholm scores and IKDC scores of the observation group were higher than those of the control group after 3,6 and 9 months of the operation(P<0.05).After 3,6 and 9 months of the operation,the passive activity detection thresholds in the observation group were lower than those in the control group(P<0.05).After 3 and 6 months of the operation,the passive angle regeneration test in the observation group was lower than that in the control group(P<0.05).Conclusion Through routine rehabilitation training combined with Zhuang medicine lotus
