摘  要:目的 探讨血清心肌肌钙蛋白Ⅰ(cT nI)、内皮素-1(ET-1)联合缺氧诱导因子-1α(HIF-1α)对射血分数保留型心力衰竭(HFp EF)的预测价值。方法 选择2018年3月至2020年2月温州市中医院收治的肺动脉高压患者103例,根据随访12个月期间患者是否发生HFp EF分为观察组(26例)和对照组(77例)。检测并比较两组患者的血清cT nI、ET-1、HIF-1α水平。采用多因素Cox比例风险回归分析法分析HFpE F的影响因素,采用ROC曲线评估血清cT nI、ET-1联合HIF-1α对HFp EF的预测价值。结果 观察组血清cT nI、ET-1、HIF-1α水平均高于对照组,差异均有统计学意义(均P<0.05)。肺动脉高压及血清cT nI、ET-1、HIF-1α水平升高均是HFpE F的危险因素(均P<0.05)。血清cT nI、ET-1、HIF-1α联合预测的特异度与AUC高于各指标单项预测(P<0.05)。结论 血清cT nI、ET-1联合HIF-1α有助于预测肺动脉高压患者发生HFp EF的风险。Objective To investigate the value of serum cardiac troponin Ⅰ(c Tn I) and endothelin-1(ET-1) in combination with hypoxia inducible factor-1α(HIF-1α) for predicting heart failure with preserved ejection fraction(HFp EF). Methods A total of 103 patients with pulmonary hypertension admitted to Wenzhou Hospital of Traditional Chinese Medicine from March 2018 to February 2020 were selected and divided into observational group(26 cases) and control group(77 cases) according to whether HFp EF occurred during 12-month follow-up. Serum c Tn I, ET-1 and HIF-1α were measured and compared between the two groups. Multivariate Cox proportional hazards regression analysis was used to analyze the influencing factors of HFp EF. ROC curve was used to evaluate predictive value of serum c Tn I and ET-1 in combination with HIF-1α in HFp EF. Results Serum c Tn I, ET-1 and HIF-1α were significantly higher in the observational group than in the control group(all P <0.05). Pulmonary hypertension and increased c Tn I, ET-1 and HIF-1α were the risk factors of HFp EF(all P<0.05). The specificity and AUC of c Tn I and ET-1 in combination with HIF-1α in the prediction of HFp EF were higher than those of c Tn I, ET-1 or HIF-1α alone(P<0.05). Conclusion Serum c Tn I, ET-1 and HIF-1α in combination may be useful for predicting HFp EF development in patients with pulmonary hypertension.
