摘 要:目的观察体外冲击波疗法联合站桩训练治疗膝骨关节炎的临床疗效。方法将90例膝骨关节炎患者随机分成A、B、C 3组,每组30例。A组患者采用体外冲击波治疗,B组患者采用站桩训练治疗,C组患者采用体外冲击波联合站桩训练治疗,均治疗4周。分别在治疗前及治疗4周后,采用疼痛视觉模拟量表(VAS)评分、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及超声检查分别评价3组患者的膝关节疼痛程度、膝关节功能及膝关节积液深度。结果治疗4周后,A组、C组的疼痛VAS评分、WOMAC骨关节炎指数评分及膝关节积液深度均较治疗前改善,而B组仅WOMAC骨关节炎指数评分较治疗前改善(均P<0.05);C组的疼痛VAS评分、WOMAC骨关节炎指数评分、膝关节积液深度均低于或小于A组与B组(均P<0.05)。结论相比于体外冲击波或站桩训练单一治疗,体外冲击波联合站桩训练更能缓解膝骨关节炎患者的膝关节疼痛与肿胀,明显改善患者的膝关节功能。Objective To observe the clinical efficacy of extracorporeal shock wave therapy combined with Zhanzhuang training in the treatment of knee osteoarthritis.Methods A total of 90 patients with knee osteoarthritis were randomly assigned to three groups in terms of groups A,B,and C,with 30 cases in each group.The patients in group A were treated with extracorporeal shock wave,group B received Zhanzhuang training,and the patients in group C were treated with extracorporeal shock wave combined with Zhanzhuang training.All the three groups received a 4-week treatment.The pre-and 4-week post-treatment pain degree of knee joint,knee joint function,and depth of knee joint effusion in the three groups were evaluated by the Visual Analogue Scale(VAS)score for pain,Western Ontario and McMaster Universities(WOMAC)osteoarthritis index score,and ultrasonography,respectively.Results After 4 weeks of treatment,groups A and C exhibited the improvements of VAS score for pain,WOMAC osteoarthritis index score,and depth of knee joint effusion as compared with before treatment,whereas group B only interpreted a improvement of WOMAC osteoarthritis index score compared to before treatment(all P<0.05).Group C yielded lower scores of VAS for pain,and WOMAC osteoarthritis index,as well as a less depth of knee joint effusion as compared with groups A and B(all P<0.05).Conclusion Compared with extracorporeal shock wave or Zhanzhuang training alone,extracorporeal shock wave combined with Zhanzhuang training can preferably alleviate the knee joint pain and swelling in patients with knee osteoarthritis,and prominently ameliorate patients′knee joint function.
