中药足浴、热敷联合玻璃酸钠关节腔注射治疗中老年膝骨关节炎临床疗效及对膝关节功能和生活质量的影响

摘　　要：目的:探究中药足浴、热敷联合玻璃酸钠关节腔注射治疗中老年膝骨关节炎(Knee Osteoarthritis,KOA)的临床疗效及对膝关节功能和生活质量的影响。方法:将90例老年KOA患者随机分为观察组和对照组各45例,对照组予以玻璃酸钠关节腔注射治疗。观察组在此基础上加用中药足浴、热敷,治疗5周。比较两组治疗后临床疗效;两组治疗前后膝关节功能、疼痛评分、生活质量。结果:治疗后,观察组总有效率高于对照组(P<0.05);治疗后,两组奎森功能演算指数(Lequesne评分)、西安大略和麦克马斯特大学骨关节炎指数(Osteoarthritis Index of Western Ontario and McMaster University,WOMAC)各项评分、视觉模拟评分法(Visual Analogue Scale,VAS)评分、炎性因子指标均降低(P<0.05),观察组均低于对照组(P<0.05);治疗后两组36项健康调查简表(36 Short Form Health Evaluation Scale,SF-36)评分均提高(P<0.05),观察组高于对照组(P<0.05)。结论:中药足浴、热敷联合玻璃酸钠关节腔注射治疗中老年KOA能提高治疗效果,缓解疼痛,改善膝关节功能,提高生活质量,值得推广。Objective:To explore the clinical efficacy of TCM medicine foot bath and hot compress plus sodium hyaluronate intra-articular injection on middle-aged and elderly knee osteoarthritis(KOA)and its influences on knee function and quality of life.Methods:A total of 90 elderly patients with KOA were randomly divided into the observation group and the control group(with 45 cases in each group).The control group was treated by sodium hyaluronate intra-articular injection.The observation group was treated by TCM medicine foot bath and hot compress more.The treatment in both groups lasted for 5 weeks.Results:After treatment,the total efficiency in the observation group was higher than the control group(P<0.05).After treatment,Lequesne score,WOMAC scores,VAS score and inflammatory factor indicators in the two groups were decreased(P<0.05),and those in the observation group were lower than the control group(P<0.05).After treatment,the SF-36 score in the two groups were increased(P<0.05),and that in the observation group was higher than the control group(P<0.05).Conclusion:TCM medicine foot bath and hot compress plus sodium hyaluronate intra-articular injection on middle-aged and elderly KOA can improve the treatment effect,relieve pain,improve knee function and quality of life,and is worthy of popularization.
