柴贝止痫汤联合西药治疗痫证临床疗效和安全性Meta分析

摘　　要：目的:系统评价柴贝止痫汤联合西药治疗癫痫的临床疗效和安全性。方法:计算机检索中国知网、维普数据库、万方数据库、中国生物医学文献数据库、PubMed、Embase等,收集关于柴贝止痫汤治疗痫证的随机对照试验,用Cochrane软件对文献进行质量评价,应用RevMan 5.4软件进行Meta分析。结果:共纳入11项研究,合计940例患者,其中对照组470例,试验组470例。相比于单用西药抗癫痫药物治疗,加用柴贝止痫汤在总有效率、发作频率、中医证候积分改善情况、认知功能、血液炎性指标以及血碱性磷酸酶、脑源性神经营养因子、神经元特异性烯醇化酶水平方面的差异有统计学意义。结论:文献证据表明,在西药治疗的基础上加用柴贝止痫汤可有效提高痫证的临床疗效,减少发作频率,改善中医证候积分,提高认知功能,减轻血液炎症反应。本研究分析尚未发现严重不良反应,但需要更多高质量文献提高证据水平。Objective:To systematically evaluate the clinical efficacy and safety of the Chaibei Zhixian decoction(柴贝止痫汤)plus Western medicine on epilepsy.Methods:Randomized controlled trials of the Chaibei Zhixian decoction plus Western medicine on epilepsy were collected by computer retrieval in the databases of CNKI,VIP,Wanfang,and CBM,PubMed,Embase.Cochrane software was used to evaluate the quality of the included literature,and a meta-analysis was performed by RevMan 5.4 software.Results:A total of 11 studies(940 patients)were included,involved 470 patients in the control group and 470 patients in the observation group.The results of the meta-analysis showed that the clinical efficacy of the Chaibei Zhixian decoction plus Western medicine treatment in treating epilepsy was superior to that of Western medicine treatment.There were significant differences between groups in the total efficiency,seizure frequency,TCM symptom score,cognitive function,blood inflammatory indices,alkaline phosphatase,brain-derived neurotrophic actor and newon specific enolase.Conclusion:The Chaibei Zhixian decoction plus Western medicine on epilepsy can improve the diagnostic effect,decrease the seizure,ameliorate TCM symptom score,improve recognition function,reduce the blood inflammatory response.No serious adverse reaction has been found in this study,but more high-quality literature is needed to raise the level of evidence.
