摘 要:目的:探讨B细胞活化因子(B Cell Activating Factor,BAFF)启动子-871C/T基因多态性与福建汉族人群热毒炽盛型系统性红斑狼疮(Systemic Lupus Erythematosus,SLE)患者的相关性。方法:入组2019年8月-2020年8月福建中医药大学附属人民医院住院及门诊热毒炽盛型SLE组患者57例及健康对照组60例,采用连接酶检测反应测序分型热毒炽盛型SLE组及健康对照组BAFF启动子序列-871C/T基因多态性并进行统计分析,χ^(2)检验比较分析两组间基因型频率及等位基因频率,Logistic回归模型分析基因型分布与热毒炽盛型SLE患者病程及疾病活动度的关系。结果:热毒炽盛型SLE组患者及健康对照组BAFF-871C/T位点存在C/C、C/T、T/T 3种基因型,热毒炽盛型SLE组患者BAFF-871C/T位点基因型分布及C、T等位基因分布频率与健康对照组的差异均有统计学意义(P <0.05)。热毒炽盛型SLE患者CC、CT、TT基因型与病程、疾病活动度均无明显相关性(P> 0.05),但在杂合基因型CT与纯合基因型CC、TT分析中发现杂合基因型与病程≥5年存在一定的相关性(P=0.042)。结论:BAFF启动子-871 C/T位点可能是福建汉族热毒炽盛型SLE人群的易感基因,可进一步为临床SLE辨证分型为热毒炽盛型提供微观参考,杂合基因型CT的热毒炽盛型SLE患者病程可能更长。Objective: To investigate the relationship between BAFF promoter-871C/T gene polymorphism and patients with severe heat-toxin syndrome SLE of Han nationality in Fujian. Methods: In this study, a total of 57 patients with severe heat-toxin syndrome SLE and 60 healthy people were inclued from August 2019 to August 2020. The BAFF promoter sequence-871C/T gene polymorphism in the SLE group and the healthy control group was genotyped by ligase detection reaction sequencing, and related statistical analysis was performed. The genotype frequency and allele frequency between the two groups was analyzed by Chi-square test. Logistic regression model was used to analyze the relationship between the genotype distribution and the disease course and disease activity of patients with severe heat-toxin syndrome SLE. Results: There were 3 genotypes of C/C, C/T and T/T at the BAFF-871C/T locus in the SLE group and the healthy control group. The genotype distribution of BAFF-871C/T locus and the distribution frequency of C and T alleles in the SLE group were significantly different from those in the healthy control group(P<0.05). There were no significant correlation between CC, CT and TT genotypes and disease duration and disease activity of patients with severe heat-toxin syndrome SLE. However, when analyzing the heterozygous genotype CT and the homozygous genotype CC and TT, it was found that there was a certain correlation between the heterozygous genotype and the disease course of not less than 5 years(P=0.042). Conclusion: The BAFF promoter-871 C/T locus may be a susceptibility gene for patients with severe heat-toxin syndrome SLE in Han nationality in Fujian, and it may further provide a microscopic reference for clinical SLE syndrome differentiation and classification of severe heat-toxin syndrome. The severe heat-toxin syndrome SLE patients with heterozygous CT genotype may have a longer disease duration.
