针刀松解联合芒针刺腰夹脊穴治疗腰椎间盘突出症的临床研究

摘　　要：目的:探讨针刀松解联合芒针刺腰夹脊穴治疗腰椎间盘突出症(Lumbar Disc Herniation,LDH)的临床疗效。方法:选择2019年4月-2021年2月于徐州市贾汪区人民医院就诊的LDH患者64例,根据随机原则分为试验组(32例)与对照组(32例),对照组患者针刀松解法治疗,试验组患者在此基础上联合芒针刺腰夹脊穴治疗,比较两组患者的腰椎功能及生活质量。结果:试验组患者治疗后的麦吉尔疼痛问卷(McGill Pain Questionnaire,MPQ)评分、Oswestry功能障碍指数(Oswestry Disability Index,ODI)评分低于对照组(P <0.05);试验组患者治疗后的日本骨科协会评估治疗分数(Japanese Orthopaedic Association Scores,JOA)高于对照组(P <0.05);试验组患者治疗后的腰腿疼痛、肢体麻木、肢体沉重、腰膝酸软积分均低于对照组(P <0.05);试验组患者治疗后的急性疼痛发作次数、持续时间均少于对照组(P <0.05);试验组患者治疗后的36项简明健康状况评价量表(36-item Short Form Health Assessment Scale,SF-36)8个维度得分均高于对照组(P <0.05)。结论:针刀松解联合芒针刺腰夹脊穴治疗可有效减轻LDH患者的腰腿疼痛症状,改善患者的腰椎功能,从而提高患者的生活质量。Objective: To investigate the clinical effect of acupotomy plus awn needle acupuncture at Jiaji point(EX-B2) at the waist on LDH. Methods: A total of 64 cases from April 2019 to February 2021 were selected and randomly divided into the test group(32 cases)and the control group(32 cases). The patients in the control group were treated by acupotomy, and the patients in the test group were given additionally awn needle acupuncture at Jiaji point, at the waist. Results: The McGill score and ODI score in the test group after treatment were lower than those in the control group(P<0.05). The JOA score in the test group after treatment was higher than the control group(P<0.05). The scores of waist and leg pain, limb numbness, heaviness of limbs, and soreness and weakness of waist and knees in the test group were lower than the control group(P<0.05). After treatment, the number and duration of acute pain attacks in the test group was less than the control group(P<0.05). The scores of 8 dimensions of SF-36 scale in the test group after treatment were higher than the control group(P<0.05). Conclusion: Acupotomy plus awn needle acupuncture at Jiaji point at the waist can effectively relieve waist and leg pain of LDH patients, improve the lumbar spine function, and thus improve quality of life of the patient.
