基于“久病入络”理论探讨熊明炤主任经验方治疗儿童腺样体肥大的临床观察

摘　　要：目的:观察熊明炤主任经验方治疗儿童腺样体肥大的临床疗效,并探讨“久病入络”理论与腺样体肥大病机的关联。方法:将80例儿童腺样体肥大患者随机分为治疗组与对照组各40例。治疗组予中药(熊明炤主任经验方)加减口服,对照组予糠酸莫米松鼻喷雾剂喷鼻联合孟鲁司特钠咀嚼片口服,均连续治疗4周。观察比较两组患者的主症积分、血瘀证总积分、腺样体体积变化情况及两组的临床疗效。结果:治疗后两组患者主症(鼻塞、打鼾、张口呼吸)积分、血瘀证(目下黑、面色晦暗不泽、舌下脉络青紫、舌质暗红或有瘀斑、脉涩)总积分及腺样体体积积分均较治疗前下降(P <0.05)。治疗组总有效率明显高于对照组。结论:熊明炤主任经验方治疗小儿腺样体肥大疗效显著,小儿腺样体肥大多与气滞血瘀、气虚血瘀及久病入络有关,值得进行深入研究和临床推广。Objective:To observe the clinical efficacy of Director XIONG Mingzhao’s empirical prescription on adenoid hypertrophy in children,and explore the relationship between the theory of“Long illness enters the collaterals”and the pathogenesis of adenoid hypertrophy.Methods:A total of 80 cases were randomly divided into the treatment group and the control group with 40 cases in each.The treatment group was given Chinese medicinal material(empirical prescription from Director XIONG Mingzhao)orally,while the control group was given mometasone furoate nasal spray plus montelukast sodium chewable tablet orally.All the included patients were treated continuously for 4 weeks.Results:After treatment,in both groups,the scores of main symptoms(stuffy nose,snoring,and mouth breathing),total score of blood stasis syndrome(black eyes,dull complexion,bruised sublingual veins,dark red tongue or ecchymosis,astringent pulse)and adenoid volume scores were significantly lower than those before treatment(P<0.05).The total efficiency in the treatment group was significantly higher than that in the control group.Conclusion:The empirical prescription from Director XIONG Mingzhao shows significant curative effect on adenoid hypertrophy in children.Adenoid hypertrophy in children are mainly related to Qi(气)stagnation and blood stasis,Qi deficiency and blood stasis,and chronic illness.It is worthy of in-depth research and clinical promotion.
