“秩边透水道”配合隔盐灸治疗卒中后尿失禁的临床研究

摘　　要：目的:观察芒针“秩边透水道”配合隔盐灸神阙穴治疗卒中后尿失禁的临床疗效。方法:将90例卒中后尿失禁患者随机分为对照组和试验组,每组45例。对照组予以常规针刺治疗,试验组在对照组基础上加芒针“秩边透水道”配合隔盐灸神阙穴治疗。观察两组组内治疗前后及两组组间治疗后的膀胱最大容量、残余尿量、膀胱过度活动症评分问卷(Overactive Bladder Syndrome Score,OABSS)及日常生活能力评分(Modified Barthel Index,MBI)的变化情况。结果:两组治疗后在膀胱最大容量、残余尿量、OABSS及MBI方面与治疗前对比差异均有统计学意义(P<0.05);试验组治疗后在膀胱最大容量、残余尿量、OABSS评分方面与对照组差异均有统计学意义(P<0.05);试验组治疗后在MBI评分方面与对照组比较差异无统计学意义(P>0.05)。结论:芒针“秩边透水道”配合隔盐灸神阙穴在改善患者膀胱最大容量、残余尿量及膀胱过度活动症方面优于常规针刺,可显著提高常规针刺治疗卒中后尿失禁的疗效。Objective:To observe the clinical effect of acupuncture at“Zhibian(BL54)through Shuidao(ST28)”plus salt-separated moxibustion at Shenque(RN8)in the treatment of urinary incontinence after stroke.Methods:Ninety patients with urinary incontinence after stroke were randomly divided into the control group and the experimental group,with 45 cases in each group.The control group was given routine acupuncture treatment,while the experimental group was treated with acupuncture at“Zhibian(BL54)through Shuidao(ST28)”plus salt-separating moxibustion at Shenque(RN8)more.The changes of maximum bladder capacity,residual urine volume,overactive bladder syndrome score questionnaire(OABSS)and Modified Barthel Index(MBI)in the two groups before and after treatment and between the two groups were observed.Results:There were significant differences in the maximum bladder capacity,residual urine volume,OABSS and MBI between the two groups after treatment compared with those before treatment(P<0.05).After treatment,the maximum bladder capacity,residual urine volume and OABSS score in the experimental group were significantly different from those in the control group(P<0.05);there was no significant difference in the MBI score between the experimental group and the control group after treatment(P>0.05).Conclusion:Mushroom acupuncture at“Zhibian(BL54)through Shuidao(ST28)”plus salt-separated moxibustion at Shenque(RN8)is superior to conventional acupuncture in improving patients’maximum bladder capacity,residual urine volume and overactive bladder,and can significantly improve conventional acupuncture in the treatment of post-stroke urinary incontinence.
