凉血消风散结合放血疗法治疗血热生风型神经性皮炎的临床疗效观察

摘　　要：目的:观察凉血消风散结合放血疗法治疗血热生风型神经性皮炎患者的临床效果,以及对相关血清炎性因子的影响。方法:将146例符合诊断标准的神经性皮炎患者随机分为综合治疗组和西药对照组,每组各73例。西药对照组患者口服西替利嗪片,并结合患处涂抹糠酸莫米松软膏;综合治疗组患者口服凉血消风散结合皮损处放血疗法治疗,两组患者治疗时间均为30 d。对两组患者的临床疗效进行观察,并比较两组患者皮损处的湿疹面积及严重度指数(Eczema Area and Severity Index,EASI)评分,健康调查12条简表(12-items Short Form Health Survey,SF-12)评分,中医证候评分和临床疗效;并在治疗结束后3个月、6个月进行随访。血常规检测两组患者外周血清中的超敏C反应蛋白(Hypersensitive C-reactive Protein,hs-CRP)和免疫球蛋白E(Immunoglobulin E,IgE)的表达水平。结果:治疗前,两组各项指标的差异无统计学意义。治疗结束后,综合治疗组的临床疗效显著优于西药对照组(P=0.034)。治疗结束后,以及3个月和6个月的随访过程中,综合治疗组的的中医证候评分和皮损处的EASI评分也显著低于西药对照组(P <0.01)。综合治疗组的hs-CRP和IgE的水平也显著低于西药对照组(P <0.01)。结论:运用凉血消风散结合放血疗法治疗血热生风型神经性皮炎有较好的临床疗效,能显著改善患者的临床症状,降低血清中与神经性皮炎关系密切的指标,且有较好的远期疗效。Objectives:To observe the clinical effect of Liangxue Xiaofeng San(凉血消风散)plus bloodletting therapy on neurodermatitis of the blood-heat wind type,and its influence on related serum inflammatory factors.Methods:A total of 146 patients with neurodermatitis were randomly divided into the comprehensive treatment group and Western medicine control group,with 73 cases in each group.The Western medicine control group took cetirizine tablets orally and applied mometasone furoate ointment on the affected area.The comprehensive treatment group was treated with Liangxue Xiaofeng San plus bloodletting therapy,and both treated for 30 days.The clinical efficacy in the two groups was observed,and the EASI score,SF-12 score,TCM syndrome score and clinical efficacy in the two groups were compared.The expression levels of hs-CRP and IgE in peripheral serum in the two groups were detected by routine blood test.Results:Before treatment,there was no significant difference between the two groups.After treatment,the clinical effect in the comprehensive treatment group was significantly better than the Western medicine control group(P=0.034).After the treatment,and during the followed-up of 3rd and 6th month,the TCM syndrome score and EASI score of skin lesions in the comprehensive treatment group were also significantly lower than those in the Western medicine control group(P<0.01).The levels of hs-CRP and IgE in the comprehensive treatment group were also significantly lower than the Western medicine control group(P<0.01).Conclusion:Liangxue Xiaofeng San plus bloodletting therapy on neurodermatitis of the blood-heat wind type shows good clinical efficacy,can significantly ameliorate the clinical symptoms of patients,reduce the serum indicators closely related to neurodermatitis,and has good long-term efficacy.
