摘　　要：目的:探讨在儿童过敏性紫癜(Henoch-Schonlein purpura, HSP)及过敏性紫癜性肾炎(Henoch-Schonlein purpura nephritis, HSPN)中抗内皮细胞抗体(anti-endothelial cell antibody, AECA)检测的临床意义。方法:选取2018年7月至2019年4月于南京医科大学附属儿童医院就诊的30例HSP患儿(HSP组)和40例HSPN患儿(HSPN组),匹配40例择期手术患儿作为对照组,检测这些患儿血清中IgG-AECA及IgA、IgM、IgG、IL-1β、sIL-2R、IL-6、IL-10、TNF-α的表达水平。使用多元线性回归模型分析IgG-AECA与血清免疫球蛋白及炎症因子水平的关联,将70例HSP和HSPN患儿分为IgG-AECA阳性组和IgG-AECA阴性组,分析两组间免疫球蛋白及炎症因子水平的差异。结果:HSP组、HSPN组和对照组间IgG-AECA阳性率差异有统计学意义(P<0.05);IgG-AECA阳性组患儿血清中IgA、IgM、IgG、IL-1β、sIL-2R、IL-6、IL-10和TNF-α水平均高于IgG-AECA阴性组(P<0.05)。结论:IgG-AECA对HSP及HSPN的诊断具有临床参考意义。Objective: To investigate the clinical significance of serum anti endothelial cell antibody detection in children with Henoch-Schonlein purpura(HSP) and Henoch-Schonlein purpura nephritis(HSPN). Methods: A total of 30 children with HSP and 40 children with HSPN treated in Children’s Hospital of Nanjing Medical University from July 2018 to April 2019 were randomly selected as study group, and 40 children with elective surgery were matched as normal control group. The expression level of IgG-AECA and immunoglobulin and inflammatory factors IgA, IgM, IgG, IL-1β, sIL-2 R, IL-6, IL-10 and TNF-α in serum of 110 children was detected. The association of IgG-AECA with immunoglobulin and inflammatory factors was analyzed using multiple linear regression models. Further, 70 children were divided into IgG-AECA positive group and IgG-AECA negative group, and the differences of immunoglobulin and inflammatory factors between the two groups were analyzed. Results: There was a significant difference in IgG-AECA positive rate among the HSP group, the HSPN group and the control group(P<0.05). The levels of IgA, IgM, IgG, IL-1β, SIL-2 R, IL-6, IL-10 and TNF-α in IgG-AECA positive group were higher than those of IgG-AECA negative group(P<0.05). Conclusion: IgG-AECA has clinical significance in the diagnosis of HSP and HSPN.
