依降钙素联合钙尔奇D及骨化三醇在老年骨质疏松性腰背痛患者中的应用效果观察

摘　　要：目的:探讨依降钙素联合钙尔奇D及骨化三醇在老年骨质疏松性腰背痛(OLBP)患者中的应用效果。方法:选取2018年10月—2020年10月北京市石景山医院收治的老年OLBP患者121例,采用随机数表法分为观察组(n=61)和常规组(n=60)。常规组给予钙尔奇D及骨化三醇治疗,观察组在常规组治疗基础上给予依降钙素治疗,比较两组腰椎骨密度(BMD)、腰椎功能[Oswestry功能障碍指数(ODI)]、疼痛程度[视觉模拟量表(VAS)]及不良反应发生情况。结果:观察组和常规组治疗90、180 d后的腰椎BMD高于治疗前,且观察组治疗90、180d后的BMD高于常规组,差异有统计学意义(P<0.05);观察组和常规组治疗90、180 d后的ODI、VAS评分均低于治疗前,且观察组治疗90、180 d后的ODI、VAS评分低于常规组,差异有统计学意义(P<0.05);观察组和常规组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:在钙尔奇D及骨化三醇治疗基础上,联合依降钙素治疗可提高老年OLBP患者腰椎BMD,有利于改善腰椎功能及疼痛症状,且安全性好。Objective To investigate the effect of Elcatonic combined with Caltrate D and Calcitriol in elderly patients with osteoporotic low back pain(OLBP).Methods From October 2018 to October 2020,121 elderly patients with OLBP admitted to the Department of Outpatient and Ward of Beijing Shijingshan Hospital were selected,they were divided into observation group(n=61)and routine group(n=60)according to the random number table method.The routine group was treated with Caltrate D and Calcitriol,and the observation group was treated with Calcitonin on this basis.The lumbar spine bone mineral density(BMD),lumbar spine function[Oswestry disability index(ODI)],pain[visual analog scale(VAS)]and the occurrence of adverse reactions were compared between the two groups.Results The BMD of the lumbar spine after 90,180 days of treatment in the observation group and the routine group was higher than that before treatment,and the BMD in the observation group after 90,180 days of treatment was higher than that in the routine group,the difference was statistically significant(P<0.05).The ODI and VAS scores of the observation group and the routine group after 90,180 days of treatment were lower than those before treatment,and the ODI and VAS scores of the observation group after 90,180 days of treatment were lower than those of the routine group,and the difference was statistically significant(P<0.05).There was no significant difference in the adverse reaction rate between the observation group and the routine group(P>0.05).Conclusions Elcatonic combined with Caltrate D and Calcitriol can improve BMD in elderly patients,and better safety.
