健脾利湿解毒膏穴位贴敷对结肠腺瘤性息肉术后患者症状缓解及病情复发的影响

摘　　要：目的:观察健脾利湿解毒膏穴位贴敷对结肠腺瘤性息肉术后患者症状缓解及病情复发的影响。方法:76例结肠腺瘤性息肉术后患者按照随机数字表法分为治疗组38例和对照组38例。对照组术后给予常规治疗和常规护理干预,治疗组在此基础上给予自拟健脾利湿解毒膏穴位贴敷。治疗12周后,比较两组患者治疗后中医证候积分(腹痛、腹胀、大便性状、大便次数、便血、食少纳呆、总分)、肠鸣音恢复时间、首次排气时间、首次排便时间、住院时间、临床疗效及术后复发状况,并记录患者治疗过程中出现的不良反应。结果:治疗后,治疗组有效率为94.7%(36/38),对照组为76.3%(29/38),两组比较,差异有统计学意义(χ^(2)=5.208,P<0.05)。治疗后,两组患者腹痛、腹胀、大便性状、大便次数、便血、食少纳呆积分及总分均显著低于治疗前(P<0.05),且治疗组以上中医证候积分及总分均显著低于同期对照组(P<0.05)。治疗组肠鸣音恢复时间、首次排气时间、首次排便时间及住院时间均明显短于对照组(P<0.05)。治疗组术后1年复发率为15.79%(6/38),对照组为34.21%(13/38),两组比较,差异无统计学意义(χ^(2)=3.439,P>0.05);治疗组术后两年复发率为42.1%(16/38),对照组为68.4%(26/38),两组比较,差异有统计学意义(χ^(2)=5.322,P<0.05)。两组治疗期间均未出现与药物相关的严重不良反应。结论:健脾利湿解毒膏穴位贴敷能明显缓解结肠腺瘤性息肉患者术后症状,降低术后复发率,提高治疗效果,且安全可靠。Objective:To observe the effect of acupoint application of Jianpi Lishi Jiedu Ointment on symptom relief and disease recurrence in patients with colonic adenomatous polyps after surgery.Methods:76 patients with colonic adenomatous polyps were randomly divided into a treatment group of 38 cases and a control group of 38 cases according to the random number table method.The control group was given routine treatment and routine nursing intervention after operation,and the treatment group was given self-made Jianpi Lishi Jiedu Ointment acupoint sticking therapy on this basis.After 12 weeks of treatment,the TCM syndrome scores(abdominal pain,abdominal distension,stool characteristics,stool frequency,blood in the stool,poor appetite,total score),recovery time of bowel sounds,time to first exhaust,and first defecation were compared between the two groups after treatment.The time,hospital stay,clinical efficacy and postoperative recurrence were recorded,and the adverse reactions occurred during the treatment of the patients were recorded.Results:After treatment,the effective rate was 94.7%(36/38)in the treatment group and 76.3%(29/38)in the control group,and the difference was statistically significant(χ^(2)=5.208,P<0.05).After treatment,abdominal pain,abdominal distension,stool character,stool frequency,blood in stool,poor appetite and total score of the two groups were significantly lower than those before treatment(P<0.05).The recovery time of bowel sounds,the time to the first exhaust,the time to the first defecation and the hospital stay in the treatment group were significantly shorter than those in the control group(P<0.05).The recurrence rate at 1 year after operation was 15.79%(6/38)in the treatment group and 34.21%(13/38)in the control group.There was no significant difference between the two groups(χ^(2)=3.439,P>0.05).The annual recurrence rate was 42.1%(16/38),compared with 68.4%(26/38)in the control group,and the difference was statistically significant(χ^(2)=5.322,P<0.05).There were no serious drug-relat
