摘　　要：目的亚急性甲状腺炎(SAT)是一种自限性的甲状腺炎症性疾病,多由呼吸道病毒感染所致,不典型症状的SAT通过彩超易于造成误诊。本文通过检测甲功和NLR等血液检验指标,评估其在SAT疾病中诊断和治疗方面的应用价值。方法选取我院138例SAT患者,分别检测急性炎症期和恢复期的FT_(3)、FT_(4)、TSH、ESR、WBC、Neu、Lym和NLR水平。采用配对t检验进行统计学分析。结果SAT急性炎症期FT_(3)、FT_(4)和TSH甲状腺功能指标、ESR,以及全血细胞分析中的WBC、Neu、Lym和NLR,相较于恢复期差异均具有统计学意义(P<0.01)。与SAT急性炎症期各指标性别间无差异不同的是,在SAT恢复期男性患者相较女性患者,具有较高的TSH水平和较低的Neu、NLR水平(P≤0.05)。结论甲状腺功能、ESR、WBC、NLR等指标在SAT急性炎症期和恢复期的显著变化,可为疾病的诊断,急性期的随访和治疗方案的选择提供客观依据。Objective Subacute thyroiditis(SAT)is a self-limited thyroid inflammatory disease,which is mostly caused by respiratory virus infection.SAT with atypical symptoms is easy to be mis-diagnosed by color ultrasound.The purpose of this study is to evaluate the application value of thyroid function and NLR in the diagnosis and treatment of SAT disease.Methods Levels of FT_(3),FT_(4),TSH,ESR,WBC,Neu,Lym and NLR among study subjects were measured in acute inflammatory phase and recovery stage of SAT in our hospital.Paired t-test was conducted to evaluate statistical difference.Results The thyroid function indexes of FT_(3),FT_(4) and TSH,ESR,and WBC,Neu,Lym and NLR in whole blood cell analysis of SAT in acute inflammatory stage were significantly different from those in recovery stage.Compared with female patients in the acute inflammatory stage of SAT,male patients in the recovery stage of SAT had higher levels of TSH and lower levels of Neu and NLR.Conclusion Significant changes of thyroid function,ESR,WBC and NLR in the acute inflammatory phase and recovery stage of SAT can provide an objective reference basis for the diagnosis,follow-up and treatment of the disease.
