摘　　要：目的探讨系统性红斑狼疮患者肾损害程度与尿轻链kappa型(U-kap)、尿轻链lambda型(U-lam)水平的关系。方法选择2019年10月至2021年10月在医院就诊的168例系统性红斑狼疮患者作为研究对象,采用肾小球滤过率估值(eGFR)评估患者肾损害程度并分为正常组、轻度组、中度组、重度组、极重度组。比较不同肾损害程度患者基线资料、实验室指标及U-kap、U-lam水平,采用Kendall’s tau-b相关系数分析U-kap、U-lam水平与肾损害程度的关系。结果极重度组、重度组、中度组及轻度组病程、血肌酐(Scr)均高于正常组,差异有统计学意义(P<0.05);极重度组、重度组、中度组Scr水平高于轻度组,且极重度组、重度组高于中度组,极重度组高于重度组,差异有统计学意义(P<0.05);不同组间性别及年龄比较,差异无统计学意义(P>0.05);极重度组、重度组、中度组、轻度组U-kap、U-lam水平均高于正常组,且极重度组、重度组、中度组高于轻度组,极重度组、重度组高于中度组,极重度组高于重度组,差异有统计学意义(P<0.05);经Kendall’s tau-b相关系数分析,U-kap、U-lam水平与系统性红斑狼疮患者肾损害程度呈正相关(r>0,P<0.05)。结论U-kap、U-lam水平与系统性红斑狼疮患者肾损害程度呈正相关。Objective To investigate the relationship between renal damage and urinary kappa light chain(U-kap)and urinary lambda light chain(U-lam)levels in patients with systemic lupus erythematosus(SLE).Methods 168 patients with systemic lupus erythematosus admitted to the hospital from October,2019 to October,2021 were selected as the research subjects.The estimated glomerular filtration rate(EGFR)was used to evaluate the degree of renal damage of patients,and patients were then divided into the normal group,mild group,moderate group,severe group and extremely severe group.Baseline data,laboratory parameters and U-kap,U-lam levels of patients with different degrees of renal damage were compared.Kendall’s tau-b correlation coefficient was used to analyze the relationship between the levels of U-kap and U-lam and the degree of renal damage.Results The course of disease and serum creatinine(Scr)of extremely severe group,severe group,moderate group and mild group were higher than those of the normal group(P<0.05).Scr levels in the extremely severe group,severe group and moderate group were higher than those in the mild group,and those in the extremely severe group,while severe group were higher than those in the moderate group,and those in the extremely severe group were higher than those in the severe group(P<0.05).There was no significant difference in gender and age among different groups(P>0.05).Levels of U-kap and U-lam in the extremely severe group,severe group,moderate group and mild group were higher than those in the normal group,and the extremely severe group,severe group and moderate group were higher than the mild group,while the extremely severe group and severe group were higher than the moderate group,and the extremely severe group was higher than the severe group(P<0.05).Kendall’s tau-b correlation coefficient analysis showed that levels of U-kap and U-lam were positively correlated with the degree of renal damage in patients with systemic lupus erythematosus(r>0,P<0.05).Conclusion Levels of U-kap and U-l
