摘  要:目的研究TRAb在甲状腺毒症初诊患者病因诊断中的价值,并确定最佳的截断值。方法回顾性研究本院首次就诊的Graves病患者270例,亚急性甲状腺炎患者196例,所有患者均接受TRAb检测。通过绘制ROC曲线,确定TRAb诊断Graves病与亚急性甲状腺炎的最佳cut-off值,并计算其灵敏度、特异性、准确度、阳性预测值和阴性预测值。结果cut-off值为1.05U/L时,诊断的灵敏度为92.59%,特异性为80.61%,阳性预测值为84.46%,阴性预测值为88.20%,诊断准确度为87.12%。结论TRAb水平为1.05U/L时,可作为甲状腺毒症患者鉴别诊断Graves病与亚急性甲状腺炎的截断值。Objective To study the level of TRAb in newly diagnosed patients with thyrotoxicosis.Methods A retrospective study was conducted in 270 patients with Graves′disease and 196 patients with subacute thyroiditis.All patients were tested for TRAb.By drawing ROC curve,the optimal cut-off value of TRAb in the diagnosis of Graves′disease and subacute thyroiditis was determined,and its sensitivity,specificity,accuracy,positive predictive value and negative predictive value were calculated.Results When the cut-off value of the Graves′disease group and subacute thyroiditis group was 1.05U/L,all related measurements were as following:the sensitivity was 92.59%,the specificity was 80.61%,the positive predictive value was 84.46%,the negative predictive value was 88.20%,and the diagnostic accuracy was 87.12%.Conclusion TRAb level of 1.05U/L can be used as a reference index for the differential diagnosis of Graves′disease and subacute thyroiditis.
