经皮椎间孔镜治疗各型腰椎间盘突出症的临床疗效评定

摘　　要：目的:比较经皮椎间孔镜技术治疗中央型、旁中央型、旁侧型、极外侧型腰椎间盘突出症的临床疗效。方法:106例经CT和MRI确诊的腰椎间盘突出症患者按照其影像学诊断分为中央型28例(26.42%),旁中央型25例(23.58%),旁侧型30例(28.30%),极外侧型23例(21.70%)。均采用经皮椎间孔镜手术治疗,随访6个月,观察下腰痛JOA评分进行疗效评定。结果:总有效率:中央型96.43%,旁中央型96.00%,旁侧型96.67%,极外侧型100%;总改善率:中央型54.82%,旁中央型72.43%,旁侧型70.32%,极外侧型6.35%。结论:经皮椎间孔镜技术治疗各型腰椎间盘突出症临床疗效明确,总改善率:旁中央型最高,旁侧型优于极外侧型,中央型最低。Objective:To compare the clinical efficacy of percutaneous transforaminal nucleotomy by trans-foraminal endoscopic spine system(TESSYS)in the treatment of central,paracentral,lateral and extreme lat-eral lumbar disc hemiation.Methods:106 patients with lumbar dise herniation confirmed by CT and MRI.According to their imaging diagnosis,all patients were divided into 4 groups:central type in 28 cases(26.42%),paracentral type in 25 cases(23.58%),lateral type in 30 cases(28.30%),and extreme lateral type in 23 cases(21.70%).AIll cases were treated with percutaneous transforaminal nucleotomy by German Spinendos TESSYS and followed-up for 6 months after operation,and also the JOA score of low back pain was observed and the curative effect was evaluated.Results:The total effective rate:central type 96.43%,paracentral type 96.00%,lateral type 96.67%,extreme lateral type 100%.Overall improvement rate:central type 54.82%,paracentral type 72.43%,lateral type 70.32%,extreme lateral type 66.35%.Conclusion:The elinical efficacy of German Spinendos TESSYS in the treatment of various types of lumbar disc herniation is clear,the total improvement rate:paracentral type is the highest,lateral type is better than extreme lateral type,central type is the lowest.
