摘 要:目的 研究晚期早产儿出生后不同时机添加维生素D对其矫正月龄6个月时的身长、体重、头围、25羟维生素D[25(OH)D]、骨密度的影响。方法 选取2020年7月至2021年12月该院收住新生儿病房的胎龄34~<37周晚期早产儿120例,采用随机对照研究方法分为出生后3 d添加维生素D制剂组和出生后1周添加维生素D制剂组,每组60例。纯母乳喂养或母乳和配方奶混合喂养的晚期早产儿口服维生素D3 800IU/d;完全配方奶粉喂养的晚期早产儿口服维生素D3 400 IU/d。结果 2组早产儿出生时,矫正胎龄40周、1个月时身长、体重、头围、25(OH)D水平比较,差异均无统计学意义(P>0.05);2组早产儿矫正月龄3~6个月时身长、体重、头围、25(OH)D水平比较,差异均有统计学意义(P<0.05);2组早产儿矫正月龄3个月时骨密度比较,差异无统计学意义(P>0.05),矫正月龄6个月时骨密度比较,差异有统计学意义(P<0.05)。结论 晚期早产儿出生后3 d尽早添加适量的维生素D制剂可提高早产儿体格发育、维生素D水平,增强骨密度;对早产儿追赶发育、提高健康水平具有重要意义。Objective To study the effects of vitamin D supplementation at different time after birth on the length,body weight,head circumference,25-hydroxyvitamin D[25(OH)D]and bone mineral density(BMD)of late preterm infants at the corrected age of six months.Methods From July 2020 to December 2021,a total of 120 late preterm infants with gestational age of 34 to less than 37 weeks admitted to the neonatal ward of this hospital were randomly divided into vitamin D preparation group three days after birth and vitamin D preparation group one week after birth with 60 cases in each group.Oral vitamin D 3800 IU/d for late preterm infants fed exclusively breast milk or mixed breast milk and formula milk;Oral vitamin D 3400 IU/d for late preterm infants fed with complete formula milk powder.Results There were no significant differences in the height,weight,head circumference,and 25(OH)D level between the two groups of premature infants at birth,corrected gestational age of 40 weeks and one month respectively(P>0.05).There were statistically significant differences in height,weight,head circumference and 25(OH)D level at 3-6 months corrected age(P<0.05).The BMD of the two groups of premature infants at 3 months corrected age was compared,the difference was not statistically significant(P>0.05);the bone mineral density at the corrected age of 6 months,the difference was statistically significant(P<0.05).Conclusion Adding proper amount of vitamin D preparations as early as three days after birth can improve the physical development,vitamin D level and bone mineral density of premature infants.It is of great significance for premature infants to catch up with development and improve their health.
