经椎间孔入路内镜治疗高位腰椎间盘突出症疗效分析

摘　　要：目的:回顾性分析经椎间孔入路内镜治疗高位腰椎间盘突出症的疗效。方法:分析2019年1月至2021年1月收治的高位腰椎间盘突出症患者80例病例资料。记录手术时间等围术期资料,用疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)、改良MacNab标准对临床疗效进行评估。结果:80例患者全部随访,随访时间为12~25个月,平均为(14.9±0.5)个月;手术时间为(55.00±14.37)min;术后无神经根损伤、断裂,无硬膜囊撕裂等严重并发症;4例患者术后复发,予以手术治疗。术后12个月随访,改良MacNab标准优良率为92.5%,其中优58例,良16例,可6例。随时间推移,两组VAS、ODI评分均减小,在不同时间点差异有统计学意义(P<0.05)。结论:经椎间孔入路内镜治疗高位腰椎间盘突出症是微创、安全、有效的方法之一。Objective:To retrospectively analyze and study the clinical efficacy of transforaminal endoscopic on the treatment of upper lumbar disc herniation.Methods:80 patients with upper lumbar disc herniation from January 2019 to January 2021 were selected.The perioperative data were recorded,and the clinical efficacy was evaluated by pain visual analogue scale(VAS),Oswestry disability index(ODI),and modified MacNab.Results:A total of 80 patients with upper lumbar disc herniation were followed up from 12 to 25 months,with(14.9±0.5)months on average,and the operation time was(55.00±14.37)min.There was no nerve root injury,rupture,dural sac tear and other serious complications after operation.4 patients relapsed and was treated with surgery.The modified MacNab criteria at the last follow-up showed that:excellent in 58 cases,good in 16 cases,and fair in 6 cases,with an excellent and good rate of 92.5%.With the passage of time,the VAS,and ODI scores of the two groups decreased,and the difference was statistically significant at different time points(P<0.05).Conclusion:Percutaneous endoscopic translaminar discectomy on the treatment of upper lumber disc herniation is one of the minimally invasive,safe and effective methods.
