病情针对性护理质量评价指标体系在重症胰腺炎患者中的应用效果

摘　　要：目的建立针对重症胰腺炎的护理质量评价指标体系。方法基于Donabedian模型,通过理论研究、课题组内讨论及专家函询等方式,建立针对重症胰腺炎的护理质量评价指标体系。结果重症胰腺炎护理质量评价指标包含结构、过程、结果指标3个一级质量评价指标,以及15个二级质量评价指标和51个三级质量评价指标。开展2轮专家函询,量表的2轮有效回收率分别为90.00%和85.00%;权威程度分别为0.880和0.920;协调程度的肯德尔和谐系数分别为0.268和0.203。各级、各项指标的单排序和总排序的一致性比率均<0.1,证明专家函询结果和权重设置比较可靠。评价指标实施前后的专科护理技术操作规范达标率分别为85.00%(102/120)和95.83%(115/120),重症胰腺炎护理措施执行达标率分别为86.67%(104/120)和94.16%(113/120),病情评估内容的达标率分别为89.17%(107/120)和96.67%(116/120),患者的满意率分别为74.29%(52/70)和91.43%(64/70),上述比较均有统计学差异(P均<0.05)。结论病情针对性护理质量评价指标体系的建立过程保证了科学性、客观性与可靠性,彰显了重症胰腺炎的专病护理特色,能够为重症胰腺炎针对性护理工作的开展提供一定的借鉴和参考作用。Objective To establish the nursing quality evaluation index system for severe pancreatitis.Methods Based on Donabedian’s model,a nursing quality evaluation index system for severe pancreatitis was established through theoretical research,group discussion,and expert correspondence.Results The nursing quality evaluation indexes for severe pancreatitis included 3 firstlevel quality evaluation indexes(structure,process,and outcome) and 15 second-level quality evaluation indexes,and 51 third-level quality evaluation indexes.Two rounds of expert correspondence were carried out,and the effective recovery rates of the scale in the two rounds were 90.00% and85.00%,respectively.The degrees of authority were 0.880 and 0.920,respectively.The Kendall’s coefficients of concordance were 0.268 and 0.203,respectively.The consistency ratios of single ranking and total ranking of all levels and indexes were less than 0.1,which proved that the results of expert correspondence and weight settings are relatively reliable.The compliance rates of specialized nursing procedure standard before and after the implementation of evaluation indexes were 85.00%(102/120)and 95.83%(115/120),respectively. The compliance rates of nursing measures for severe pancreatitis were 86.67%(104/120) and 94.16%(113/120),respectively.The compliance rates of disease assessment content were 89.17%(107/120) and 96.67%(116/120),respectively.The patient satisfaction rates were 74.29%(52/70) and 91.43%(64/70),respectively.The differences were statistically significant(all P <0.05).Conclusion The establishment of the evaluation index system of condition-specific nursing quality ensures the scientificity,objectivity,and reliability,highlights the specialized nursing characteristics of severe pancreatitis,and provides references for the development of targeted nursing care for severe pancreatitis.
