通闭益气活血汤联合动脉切开取栓术治疗血栓闭塞性脉管炎临床研究

摘　　要：目的:观察通闭益气活血汤联合动脉切开取栓术治疗血栓闭塞性脉管炎的临床疗效。方法:将100例血栓闭塞性脉管炎患者随机分为研究组与对照组各50例,对照组采取单纯动脉切开取栓术治疗,研究组采取动脉切开取栓术联合通闭益气活血汤治疗。比较2组治疗前后全血黏度(BV)、血浆黏度(PV)、红细胞压积(Hct)、红细胞沉降率(ESR)、红细胞聚集指数(AI)、红细胞变形指数(RCD)、全血还原黏度(BRV),并比较2组临床疗效及不良反应发生情况。结果:研究组总有效率98.0%,高于对照组88.0%(P<0.05)。与同组治疗前比较,治疗后2组BV、PV、Hct、AI、BRV均降低(P<0.05),ESR、RCD均上升(P<0.05);与对照组治疗后比较,研究组治疗后BV、PV、Hct、AI、BRV均较低(P<0.05),ESR、RCD均较高(P<0.05)。研究组不良反应发生率2.0%,低于对照组14.0%(P<0.05)。结论:采用通闭益气活血汤联合动脉切开取栓术治疗血栓闭塞性脉管炎患者,可显著提高临床效果,降低患者不良反应发生情况,改善患者血液流变学指标。Objective:To observe the clinical effect of the therapy of Tongbi Yiqi Huoxue tang combined with artery thrombectomy on thromboangiitis obliterans.Methods:A total of 100 patients were randomly divided into the study group and the control group,with 50 cases in each group.The control group was treated with artery thrombectomy only,and the study group was treated with artery thrombectomy combined with Tongbi Yiqi Huoxue tang.The blood viscosity(BV),plasma viscosity(PV),hematocrit(Hct),erythrocyte sedimentation rate(ESR),aggregation index(AI)of red cell,index of red cell deformation(RCD),blood reduced viscosity(BRV)before and after treatment were compared between the two groups.The clinical effects and the adverse reactions were compared between the two groups.Results:The total effective rate was 98.0%in the study group,higher than that of 88.0%in the control group(P<0.05).After treatment,the levels of BV,PV,Hct,AI and BRV in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of ESR and RCD were increased(P<0.05);after treatment,the levels of BV,PV,Hct,AI and BRV in the study group were lower than those in the control group(P<0.05),and the levels of ESR and RCD in the study group were higher(P<0.05).The incidence of adverse reactions was 2.0%in the study group,lower than that of 14.0%in the control group(P<0.05).Conclusion:The therapy of Tongbi Yiqi Huoxue tang combined with artery thrombectomy for patients with thromboangiitis obliterans can significantly improve the clinical effect,reduce the incidence of adverse reactions,and improve the indexes of hemorheology of patients.
