浮针法治疗腰背肌筋膜疼痛综合征临床研究

摘　　要：目的:观察浮针法治疗腰背肌筋膜疼痛综合征的临床疗效及对患者腰椎功能及疼痛的影响。方法:选取97例腰背肌筋膜疼痛综合征患者作为研究对象,采用随机数字表法分为对照组(48例)与观察组(49例)。对照组给予普通针灸法治疗,观察组给予浮针法治疗。比较2组临床疗效及治疗前后中医证候积分、腰部核心肌疼痛侧表面肌电特征、腰椎功能及疼痛。结果:治疗后,观察组总有效率为87.76%,高于对照组64.58%(P<0.05)。治疗后,2组中医证候积分、现时疼痛强度(PPI)评分、疼痛分级指数(PRI)、视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)评分、Ro-land-Morris功能障碍问卷表(RMDQ)评分均较治疗前降低(P<0.05),且观察组上述指标低于对照组(P<0.05)。治疗后,2组竖脊肌、多裂肌力-位移距离(FDD)水平和髂肋肌、最长肌、多裂肌疼痛侧中位频率(MF)水平均较治疗前升高(P<0.05),且观察组上述指标高于对照组(P<0.05)。结论:浮针法治疗腰背肌筋膜疼痛综合征疗效确切,能够有效改善患者腰部核心肌疼痛侧表面肌电特征和腰椎功能,减轻患者疼痛。Objective:To observe the clinical effect of superficial needling on lumbar and back myofascial pain syndrome,and its influence on lumbar function and pain.Methods:A total of 97 patients with lumbar and back myofascial pain syndrome were selected as the study subjects and divided into the control group(48 cases)and the observation group(49 cases)by random number table method.The control group was treated with ordinary acupuncture,and the observation group was treated with superficial needling.The clinical effects,Chinese medicine syndrome scores,electromyographic characteristics of the surface of painful side of lumbar core muscle,lumbar function and pain before and after treatment were compared between the two groups.Results:After treatment,the total effective rate was 87.76%in the observation group,higher than that of 64.58%in the control group(P<0.05).After treatment,the scores of Chinese medicine syndromes,Present Pain Intensity(PPI),Pain Rating Index(PRI),Visual Analog Scale(VAS),Oswestry Dysfunction Index(ODI)and Ro-Land-Morris Dysfunction Questionnaire(RMDQ)in both groups were all decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of force-displacement distance(FDD)of erector spine and multifidus and the levels of median frequency(MF)of painful side of iliocostus,longisus and multifidus muscles were increased when compared with those before treatment,and the above levels in the observation group were higher than those in the control group(P<0.05).Conclusion:Superficial needling has an accurate therapeutic effect on lumbar and back myofascial pain syndrome,which can effectively improve the electromyographic characteristics of the surface of painful side of lumbar core muscle and lumbar function of patients,and relieve their pain.
