压力梯度袜联合足底泵在尿道重建术中的应用效果及对静脉血栓发生率的影响

摘　　要：目的探讨压力梯度袜联合足底泵在尿道重建术中的应用效果及对静脉血栓发生率的影响。方法收集2019年4月至2021年2月于北京积水潭医院进行尿道重建术的86例患者的临床资料,根据静脉血栓预防方法的不同将患者分为对照组(使用足底泵预防静脉血栓)和观察组(使用压力梯度袜联合足底泵预防静脉血栓),每组43例。比较两组患者术后1周、2周、2个月静脉血栓及下肢水肿的发生情况,术前、术后3 d的小腿周径及大腿根部周径,切口出血量,凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)和组织型纤溶酶原激活物抗原(tPA-Ag)]水平。结果术后1周,观察组患者静脉血栓、下肢水肿的发生率均低于对照组患者,差异均有统计学意义(P<0.05);术后2周、2个月,两组患者静脉血栓、下肢水肿的发生率比较,差异均无统计意义(P>0.05)。术后3 d,观察组患者的小腿及大腿根部周径均小于对照组患者,切口出血量低于对照组患者,差异均有统计学意义(P<0.05)。术后3 d,观察组患者的PT、APTT均明显长于对照组患者,tPA-Ag水平明显高于对照组患者,差异均有统计学意义(P<0.01)。结论对于进行尿道重建术的患者,压力梯度袜联合足底泵能够降低静脉血栓和下肢水肿的发生率,减少对术后小腿及大腿根部周径的影响及切口出血量,改善患者的凝血功能。Objective To investigate the application effect of pressure gradient stockings combined with foot pump in urethral reconstruction and its effect on the incidence rate of venous thrombosis.Method The clinical data of 86 patients who underwent urethral reconstruction in Beijing Jishuitan Hospital from April 2019 to February 2021 were collected.The patients were divided into control group(use foot pump to prevent venous thrombosis)and observation group(use pressure gradient stockings combined with foot pump to prevent venous thrombosis)according to different prevention methods of venous thrombosis,with 43 cases in each group.The incidence of venous thrombosis and lower extremity edema at 1 week,2 weeks,and 2 months after the operation,and the circumference of the calf and the thigh root,the amount of incision bleeding,and the indexes of coagulation function[prothrombin time(PT),activated partial thromboplastin time(APTT),and tissue-type plasminogen activator antigen(tPA-Ag)]levels before the operation and 3 days after the operation were compared between the two groups.Result One week after the operation,the incidences of venous thrombosis and lower extremity edema in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the incidence of venous thrombosis and lower extremity edema between the two groups(P>0.05).On the third day after the operation,the circumferences of the calf and thigh root of the observation group were smaller than those of the control group,the incision blood loss volume was less than that of the control group,and the differences were statistically significant(P<0.05).On the third day after the operation,the PT and APTT of the observation group were significantly longer than those of the control group,the tPA-Ag level was significantly higher than that of the control group,and the differences were statistically significant(P<0.01).Conclusion Gradient compression stockings combined with f
