摘　　要：目的探讨迷走右锁骨下动脉(ARSA)患者出现吞咽功能障碍的危险因素。方法收集2012年4月至2020年5月德阳市中西医结合医院收治的37例ARSA患者的临床资料,其中,合并吞咽障碍症状20例。采用最小绝对收缩与选择算子(LASSO)技术进行单因素分析,筛选出可疑影响因素,然后对可疑影响因素进行多因素Logistic回归模型分析,得出独立危险因素。结果LASSO单因素分析结果显示,年龄、斑块、高血压均为ARSA患者出现吞咽功能障碍的可疑危险因素(P<0.05)。多因素分析结果显示,合并斑块是ARSA患者出现吞咽功能障碍的独立危险因素(P<0.05)。结论ARSA斑块形成是出现吞咽功能障碍的独立危险因素,临床工作中需密切关注,早发现、早治疗,可帮助患者降低吞咽功能障碍的发生率,缓解患者痛苦,减轻社会医疗负担。Objective To explore the risk factor of dysphagia in patients with aberrant right subclavian artery(ARSA).Method The clinical data of 37 patients with ARSA admitted to Deyang Integrated Traditional Chinese and Western Medicine Hospital from April 2012 to May 2020 were collected,and 20 patients of them had dysphagia symptoms.The least absolute shrinkage and selection operator(LASSO)technology was used to conduct univariate analysis and select the suspicious influencing factors.Then the suspicious influencing factors were analyzed by multivariate logistic regression to select the independent risk factors.Result LASSO univariate analysis showed that age,plaque,and hypertension were suspicious risk factors for dysphagia in ARSA patients(P<0.05).The multivariate logistic analysis showed that the concomitant plaque was an independent risk factor for dysphagia in patients with ARSA(P<0.05).Conclusion Plaque formation in patients with ARSA is an independent risk factor for dysphagia.Early detection and treatment can reduce the incidence of dysphagia,relieve suffering of patients and social medical burden.
