雷火灸联合中药熏洗在前交叉韧带重建术后早期康复中的效果观察

摘　　要：【目的】探究雷火灸联合中药通络祛风散熏洗对前交叉韧带重建术(ACLR)后早期康复患者膝关节功能的影响。【方法】将99例行ACLR术后4~6周返院康复治疗的患者随机分为雷火灸组、中药熏洗组(简称熏洗组)及雷火灸联合中药熏洗组(简称联合组),每组各33例,分别给予雷火灸(由麝香、乳香、茵陈等中药组成)、中药通络祛风散熏洗(由法半夏、制川乌、制天南星等中药组成)及雷火灸联合中药熏洗治疗,疗程为1周。观察3组患者治疗期间的膝关节活动度、肿胀度、疼痛视觉模拟量表(VAS)评分、Lysholm膝关节功能评分的变化情况。【结果】(1)治疗1周后,3组患者的膝关节活动度均明显提高(P<0.001),具体而言,从治疗第2天开始,3组患者膝关节活动度均较前1 d提高(P<0.01);组间比较,联合组患者除治疗第1天外,各观察时点的膝关节活动度均明显高于雷火灸组和熏洗组(P<0.01)。(2)治疗1周后,3组患者的膝关节肿胀度均明显降低(P<0.001),具体而言,治疗第2~4天,3组患者的膝关节肿胀度均较前1 d逐渐升高(P<0.05),第5~7天均较前1 d逐渐降低(P<0.05);组间比较,联合组患者在治疗第5天的膝关节肿胀度明显低于雷火灸组和熏洗组(P<0.05)。(3)治疗第2~4天,3组患者的膝关节疼痛VAS评分均高于第1天(P<0.05),而第5~7天逐渐降低(P<0.05);组间比较,联合组患者在治疗第4天和第6天的膝关节疼痛VAS评分均明显低于雷火灸组和熏洗组(P<0.05或P<0.01)。(4)治疗1周后,3组患者的Lysholm膝关节功能评分均明显提高(P<0.001),且联合组的Lysholm膝关节功能评分略高于雷火灸组和熏洗组,雷火灸组又略高于熏洗组,组间治疗前后差值比较,差异均有统计学意义(P<0.05)。【结论】雷火灸、中药熏洗及联合治疗均能有效缓解前交叉韧带重建术后早期康复患者膝关节活动状态下疼痛、肿胀,增加膝关节活动度,有助于膝关节功能恢复,以雷�Objective To investigate the effect of thunder-fire moxibustion combined with Chinese medicine fumigation and washing on early rehabilitation of patients after anterior cruciate ligament reconstruction(ACLR).Methods A total of 99 patients who returned to hospital for rehabilitation 4-6 weeks after ACLR were randomly divided into the thunder-fire moxibustion group,Chinese medicine fumigation and washing group(referred to as the fumigation and washing group)and the thunder-fire moxibustion combined with Chinese medicine fumigation and washing group(referred to as the combined group),with 33 patients in each group.The patients in the three groups were given thunder-fire moxibustion(the moxa cone being mainly composed of Moschus,Olibanum and Herba Artemisiae Scopariae),fumigation and washing with Chinese medicine Tongluo Qufeng Powder(mainly composed of Rhizoma Pinelliae Preparatum,Radix Aconiti Preparata,and processed Rhizoma Arisaematis),and thunder-fire moxibustion combined with Chinese medicine fumigation and washing,respectively.The course of treatment lasted for one week.The changes in range of motion(ROM)of knee joint,knee swelling,pain visual analogue scale(VAS)score and Lysholm knee function score were observed in the three groups during the treatment period.Results(1)After one week of treatment,the ROM of knee joint in the three groups was significantly increased(P<0.001),and the improvement was shown that from the second day of treatment,the ROM in all of the three groups was improved compared to that on the first previous day(P<0.01).The intergroup comparison showed that the ROM in the combined group was significantly larger than that in the thunder-fire moxibustion group and the fumigation and washing group at all observation time points except for the first day of treatment(P<0.01).(2)After one week of treatment,the swelling of the knee joint in all three groups was significantly decreased(P<0.001),and the decrease was shown that from the second to fourth day of treatment,the degree of knee joint swelli
