邓高丕治疗多囊卵巢综合征的用药规律探讨

摘　　要：采用“中医方药数据多维趋势分析”软件对邓高丕教授门诊治疗多囊卵巢综合征患者病案资料进行症状集群、证型、方药分析。结果显示,共纳入处方233首,涉及中药121味,用药总频次为3015次。症状方面,最集中的症状表现为月经有血块、口干、月经色暗红、月经量少、眠差、经行腹痛、口苦、经行腰酸痛、月经周期延长、经行乳胀等;证型方面,肾虚肝郁痰湿血瘀证占比最高,其次分别为肝郁痰湿血瘀证和肾虚痰湿血瘀证;证素按频次高低排列分别为血瘀证、肝郁证、肾虚证、痰湿证、脾虚证。治疗所用方剂整体趋于寒性及功效特点以降、下行为主,使用频次最高的药对和3味药的药物组合为当归-牛膝和当归-柴胡-生地黄。用药总体以补肾益精或滋阴补肾为主,以活血化瘀、消痈散结、理气、解郁、补脾为次。分析结果提示邓高丕教授临床治疗多囊卵巢综合征时,围绕其肾虚为本,肝郁气滞、脾虚痰湿、瘀血内停为标的病机特点,治法以补肾填精为主,以理气解郁、运脾导痰、活血化瘀为辅。The software of Multidimensional Trend Analysis of Chinese Medicine Prescription Data was used to analyze the symptom cluster,syndrome type and prescription for polycystic ovary syndrome(PCOS)treated by Professor DENG Gao-Pi in the outpatient department.The results showed that a total of 233 prescriptions were included,involving 121 herbs with the total medication frequency of 3015 times.In terms of symptoms,the commonly-seen symptoms were blood clots in the menstruation,dry mouth,dark red menstruation,scanty menstruation,poor sleep,abdominal pain during menstrual period,bitter mouth,lumbar pain during menstrual period,prolonged menstruation,breast distension during menstrual period,etc..In terms of the syndrome types,kidney deficiency and liver depression complicated with phlegm-damp and blood stasis syndrome accounted for the highest percentage,and then came the syndrome of liver depression complicated with phlegmdamp and blood stasis and the syndrome of kidney deficiency complicated with phlegm-damp and blood stasis.The syndrome elements in decreasing sequence were blood stasis,liver depression,kidney deficiency,phlegmdamp,and spleen deficiency.The prescriptions for the treatment of PCOS were characterized by being cold in nature,and mainly had the actions of descending and going downward.The most frequently-used herbal pair and3-herb combination were Radix Angelicae Sinensis-Radix Achyranthis Bidentatae,and Radix Angelicae SinensisRadix Bupleuri-Radix Rehmanniae.The therapteutic principles for PCOS is to tonify kidney and replenish essence or to nourish yin and tonify kidney,supplemented by activating blood and removing stasis,dissolving abscesses and dissipating mass,regulating qi,relieving depression,and replenishing spleen.The results indicated that for the treatment of PCOS,Professor DENG Gao-Pi focuses on the pathogenesis being deficiency of kidney in the origin,and with the superficiality characterized by liver depression and qi stagnation,spleen deficiency and phlegm-damp,and internal retention of bloo
