《医纲总枢》伤寒六经传变学术思想探析

摘　　要：有关伤寒六经传变的方式和原因,是历代注家争论的热点。清末岭南中西汇通医家陈珍阁在其《医纲总枢》中亦涉及对伤寒六经传变的认识。陈珍阁所理解的伤寒为狭义伤寒的概念,而非一切外感热病的总称;书中“附辨古传论伤寒证传经之谬”篇,批判了医家在治疗伤寒时,忽视内伤因素,治法方药使用不当,导致疾病的传变。陈珍阁的观点,是他在特殊历史时期下,面对西洋医学的冲击,进行的理论思考与探索,对于临床六经病的诊治,特别是对岭南地区六经病,具有一定的指导意义和参考价值。The ways and reasons for the transmission and change among the six meridians during exogenous cold attack are the hot points of debate among the commentators for traditional Chinese medicine theory in past dynasties.In the book of Yi Gang Zong Shu(Essentials for General Clinical Medicine),which was written by CHEN Zhen-Ge,a physician being famous for the confluence of Chinese and Western medicine in the Lingnan area in the late Qing Dynasty,the thoughts of six-meridian transmission and change of exogenous cold attack were also discussed.In the opinion of CHEN Zhen-Ge,Shanghan refers to the its narrow sense of exogenous cold attack but not its generalized conception of exogenous febrile disease.In the chapter Attached to Differentiating the Fallacy of Cold Exogenous Diseases Transmission in Ancient Biographies,CHEN Zhen-Ge criticized the physicians who neglected the internal injury factors and adopted the wrong prescriptions during the treatment of exogenous cold attack,which finally led to the transmission and change of the diseases.CHEN’s viewpoint arose from his theoretical thinking and exploration when facing the impact of western medicine in the special historical period,which has certain guiding significance and reference value for the diagnosis and treatment of the six-meridian diseases in the clinic,especially for the six-meridian diseases in Lingnan region.
