高血压合并青光眼患者中医证型与眼血流动力学及房水中IL-6、VEGF水平关系

摘　　要：目的:探讨高血压合并青光眼患者中医证型与眼血流动力学及房水中白细胞介素6(IL-6)、血管内皮生长因子(VEGF)水平的相关性。方法:选取2019年1月至2021年12月本院收治的高血压合并青光眼患者148例为研究对象,根据中医证型辨证标准分为肝火亢盛证、阴虚阳亢证、痰湿壅盛证和阴阳两虚证4型,比较不同中医证型患者眼血流动力学指标及房水中IL-6、VEGF含量,并探讨其相关性。结果:纳入的148例高血压合并青光眼患者经中医辨证分型,其中阴阳两虚证占比35.14%(52/148),肝火亢盛证27.03%(40/148)、痰湿壅盛证22.97%(34/148)和阴虚阳亢证14.86%(22/148)。不同中医证型高血压合并青光眼患者EDV、PSV、RI水平比较(P<0.05);其中阴阳两虚证患者EDV、PSV水平均低于其他证型患者,RI水平高于其他证型患者(P<0.05)。不同中医证型高血压合并青光眼患者房水中IL-6、VEGF含量比较(P<0.05);其中阴阳两虚证患者房水中IL-6、VEGF含量均高于其他证型患者(P<0.05)。相关性分析发现,高血压合并青光眼阴阳两虚证与EDV、PSV呈负相关,与RI及房水中IL-6、VEGF水平均呈正相关(P<0.05);其余各证型与眼血流动力学指标EDV、PSV、RI及房水中IL-6、VEGF水平均无相关性(P>0.05)。结论:不同中医证型高血压合并青光眼患者眼血流动力学指标EDV、PSV、RI及房水中IL-6、VEGF水平存在明显差异,其中阴阳两虚证与上述指标均存在一定相关性,可作为指导中医临床辨证施治的敏感指标。Objective: To investigate the correlation between TCM syndrome type and ocular hemodynamics and interleukin 6(IL-6) and vascular endothelial growth factor(VEGF) levels in patients with hypertension and glaucoma.Method:148 patients with hypertension and glaucoma admitted to our hospital from January 2019 to December 2021 were selected as the research objects. According to the syndrome differentiation criteria of traditional Chinese medicine, they were divided into four types: hyperactivity of liver fire, hyperactivity of yin and Yang, hyperactivity of phlegm and dampness and deficiency of yin and Yang. The ocular blood flow mechanical indexes and the contents of IL-6 and VEGF in aqueous humor of patients with different TCM syndrome types were compared and their correlations were discussed. Results: 148 cases of hypertension complicated with glaucoma were classified according to TCM syndrome differentiation, of which Yin and yang deficiency syndrome accounted for 35.14%(52/148), liver fire hyperactivity syndrome 27.03%(40/148), phlegm dampness hyperactivity syndrome 22.97%(34/148)and yin deficiency yang hyperactivity syndrome 14.86%(22/148). Comparison of EDV, PSV and RI levels in hypertension patients with glaucoma of different TCM syndrome types(P<0.05);The levels of EDV and PSV in patients with deficiency of yin and Yang were lower than those in patients with other syndrome types, and the levels of RI were higher than those in patients with other syndrome types(P<0.05). The contents of IL-6 and VEGF in aqueous humor of hypertension patients with glaucoma in different TCM syndromes were compared(P<0.05);The contents of IL-6 and VEGF in aqueous humor of patients with Yin Yang deficiency syndrome were higher than those of patients with other syndrome types(P<0.05). The correlation analysis showed that the deficiency of yin and Yang in hypertension with glaucoma was negatively correlated with EDV and PSV, and positively correlated with RI and the levels of IL-6 and VEGF in aqueous humor(P<0.05);The other syndrome ty
